CASE MANAGEMENT FOR GERONTOLOGY

Lack of collaboration between important stakeholders like families, caregivers, and professionals can be a very challenging issue. Open, honest, and clear communication is one important part of effective cooperation and minimizing potential for conflicts and communication skills are certainly an essential aspect of case managers, especially in geriatrics. 

I am curios to hear more about your perspective and thoughts about how health care organizations (and policy makers) might be able to address this issue. Additionally, do you think this is something that must be addressed on a system-wide basis?
