Prior to beginning work on this assignment,

• Read Chapter 7 and Chapter 8.
• Read the Avalon Health Care Organization Case Study. This resource provides the foundational information for your Week 4 Avalon Health Care Organization Case Study.

• Download and review the required Annotated Bibliography Template. You must use this template to complete your assignment.
• You may also wish to use any of the required and recommended resources throughout the course to complete this assignment.

For this assignment, use the Annotated BibliographyTemplate. Locate a minimum of eight scholarly or credible resources, five of which must come from the UAGC Library, that may be used to support your work for the Case Study assignment in Week 4 of our class:

• Prepare an annotated bibliography.
  • Create an APA reference.
    • See the APA: Formatting Your References List resource in the Writing Center for specifications.
  • Summarize in your own words the main points of the resource.
  • Explain how the resource is relevant to your Avalon Health Care Organization Case Study Paper.

The Annotated Bibliography paper

• Must be four to five double-spaced pages in length (not including title and references pages) and formatted according to APA Style as outlined in the Writing Center’s APA Formatting for Microsoft Word resource. Additionally, review the details on the UAGC Writing Center’s Annotated Bibliography page.
• Must include a separate title page with the following:
  • Title of paper in bold font
    • Space should appear between the title and the rest of the information on the title page.
  • Student’s name
  • Name of institution (The University of Arizona Global Campus)
  • Course name and number
  • Instructor’s name
  • Due date
• Must use the Annotated Bibliography Template.
• Must utilize academic voice. See the Academic Voiceresource for additional guidance.
• Must use at least eight scholarly or credible sources.
  • The Scholarly, Peer-Reviewed, and Other Credible Sources table offers additional guidance on appropriate source types. If you have questions about whether a specific source is appropriate for this assignment, please contact your instructor.
