To address the quality improvement practice gap, I would focus on identifying discrepancies between ideal practice and observed practice using a data-driven approach, which has been shown to improve patient care quality and safety (Cascini et al., 2021). One potential quality improvement practice gap in my current organization is the issue of medication errors during transitions of care. The ideal practice would involve seamless communication between healthcare providers and accurate documentation of medication histories. However, the reality often includes communication breakdowns and incomplete medication reconciliation, leading to errors that can harm patients.
This practice gap is a crucial issue in my DNP project because medication errors affect patient safety, increase healthcare costs, and compromise trust in the healthcare system. However, there is hope. Developing and implementing a standardized medication reconciliation process during care transitions could be an effective solution. Utilizing data from past incidents of medication errors will allow the team to pinpoint areas where communication fails, whether during patient handoffs or in documentation and make significant improvements. This solution has the potential to significantly reduce medication errors and improve patient safety, instilling hope and optimism in the healthcare system.
Two methods that can be instrumental in addressing this quality improvement gap are the SERVQUAL model and the use of clinical dashboards. Your expertise as healthcare professionals and quality improvement specialists is not just necessary, it is integral to this process. The SERVQUAL model is an effective tool for assessing service quality, particularly in healthcare, as it allows for the measurement of gaps in service delivery across five dimensions: reliability, assurance, tangibility, empathy, and responsiveness (Tripathi & Siddiqui, 2020). Using SERVQUAL would enable you to assess where the most significant gaps in medication reconciliation occur and how to tailor interventions accordingly. For instance, implementing rapid-response communication protocols could be prioritized if responsiveness is an issue.
Furthermore, clinical dashboards, which visually represent real-time data, are invaluable in monitoring medication reconciliation protocol adherence. These dashboards promote transparency and accountability by allowing stakeholders to track progress daily, ensuring any deviations from the standardized process are promptly addressed. These tools are in line with the principles of continuous quality improvement, ensuring that any changes implemented are evidence-based and sustainable.
