Four Spheres of Care Activity

Wellness and Disease Prevention:

Reflect on the health promotion disease prevention interventions you witnessed in your practicum site, as it relates to the social determinants of health most prevalent in your community. What did you see? What does this time mean to you as a professional nurse in your role?

During my practicum hours , I observed several health promotion and disease prevention interventions that addressed social determinants of health in our community. For example, the nurses provided education on healthy nutrition and physical activity in low-income neighborhoods and coordinated vaccination clinics for underserved populations. I also saw the efforts to connect patients with community resources for housing and food insecurity. This experience reinforced the importance of considering social, economic, and environmental factors when promoting health. As a professional and dedicated nurse, it reminds me that advocacy and education are vital and important tools in empowering patients and reducing health disparities.

Chronic Disease Management:

Reflect on the integration of interprofessional team-based care as it relates to chronic disease management in your practicum site this week. What did you see? What does this time mean to you as a professional nurse in your role?

This week, I observed the integration of interprofessional team-based care in managing chronic diseases like diabetes and hypertension. Nurses collaborated with physicians, dietitians, and pharmacists to develop individualized care plans, monitor lab results, and provide ongoing patient education. I saw how effective

communication and coordinated efforts between the nurses, doctors, and other staff members improve patient adherence and outcomes. As a professional nurse, this experience highlights my role in facilitating collaboration, monitoring progress, and serving as a patient advocate. Being part of a team ensures holistic care, addresses multiple aspects of chronic illness, and strengthens my understanding of comprehensive care management.

Regenerative and Restorative Care:

Reflect on the acute management of illnesses such as stroke, mental illness, and falls in your practicum site this week. What did you see? What does this time mean to you as a professional nurse in your role?

During my practicum hours this week, I observed acute management of illnesses such as stroke, hypertension, mental health crises, and falls. Nurses quickly assessed patient conditions, implemented emergency interventions, and coordinated with physicians and rehabilitation teams for ongoing care. For patients with mental illness, I saw de-escalation techniques and therapeutic communication used to ensure their safety. Witnessing these interventions underscored the critical role of timely assessment, intervention, and interdisciplinary collaboration. As a professional nurse, this experience reinforces the importance of vigilance, critical thinking, and patient advocacy in acute care settings. It also strengthens my confidence in responding to high-risk situations.

Hospice and Palliative Care:

Reflect on end-of-life nursing and advanced illness and hospice care in your practicum site this week. What did you see? What does this time mean to you as a professional nurse in your role?

This week in my practicum hours , I observed end-of-life care and support for patients with advanced illnesses. Nurses provided compassionate care, focusing on pain management, emotional support, and dignity in the dying process. I witnessed the importance of communication with patients and families, helping them navigate difficult decisions while respecting their wishes. This experience emphasized holistic care, including physical, emotional, and spiritual needs. As a professional nurse, it reminds me that my role extends beyond physical care to providing comfort, advocacy, and support. It has strengthened my understanding of the value of empathy, presence, and ethical decision-making in nursing practice.
