Week 2 Discussion

Read the attached article entitled Clinical Practice Guideline (Update): Adult Sinusitis and answer the questions below. A few notes about the article. It was written for the American Academy of Otolaryngology - Head and Neck Surgery Foundation and is a medical article. Normally I'd disallow this article if you used it as a reference but as this article details the process of developing practice guidelines, I've used it here. As a rule, you should always look for articles authored by nurses. I'm breaking my own rule here because this article contains a lot of good information. I'm going to focus mainly on Acute Bacterial Rhinosinusitis (ABRS) although the article gives information on diagnosis and treatment of acute viral rhinosinusitis, recurrent sinusitis, and chronic sinusitis. 

1. Why should a FNP be concerned about understanding the treatment guidelines for Acute Bacterial Sinusitis? What is the financial and personal consequences of the condition? 

2. The authors call these multidisciplinary guidelines. Do you agree with them? Who would you add or subtract from the expert panel that developed the guidelines? 

3. What are the diagnostic criteria for Acute Bacterial Rhinosinusitis? How do these differ from the criteria for acute viral rhinosinusitis, recurrent sinusitis, and chronic sinusitis?

4. Describe Grade B evidence as it relates to the treatment of ABRS.

5. Use table 3 to justify the use of watchful waiting for ABRS.

6. Use figure 1 to develop and describe your treatment plan for a patient who c/o ABRS symptoms of 2 weeks duration. 

7. What did this article teach you (if anything) about evidenced based practice? Are guidelines meant to be followed blindly?
