9000 Dissertation Survey Questionnaire

I'm not sure if you remember the letter we submitted to the Walden IRB (see the attached file below), and this is the IRB's response. My professor wants me to develop questions I can ask volunteers rather than incarcerated inmates.  

The goal is to gather the information without interviewing currently incarcerated inmates. 

The volunteers (10-30 Individuals) would come from different groups that visit inmates weekly or monthly. A few of the popular groups that visit the inmates are;  

1. Kairos Prison Ministry of Texas

2. Gateway Church Prison Ministry 

3. Prison Toastmasters Club
