Reflect on your performance in this course related to each area. Then, rate your performance using the following scale:  1 – Below Expectations; 2 – Working Toward Expectations; and 3 – Meets Expectations.  Take a moment to provide any additional details in the comments box that may be needed to explain any of the self-evaluation ratings provided. Once you are satisfied with your responses, click the “Submit” button at the bottom of the form. When the faculty member has completed her or his evaluation, you will receive an email (with an attached pdf) with a summary of your evaluation and the faculty evaluation.  This pdf file must be uploaded into the course to complete the assignment.

The completed assignment should address all of the assignment requirements, exhibit evidence of concept knowledge, and demonstrate thoughtful consideration of the content presented in the course. The writing should integrate scholarly resources, reflect academic expectations and current APA standards,
