I. PURPOSE OF THE SESSION: The purpose of this session is gather as informa6on as possible
about the client’s drug use history, treatment history, and events leading to client’s drug use.
II. OBSERVATION: Client is a 35-year-old Caucasian female with substance use disorder. Client
reported that she began using in high school and has been using since then. Client reported that her
addiction has caused her lose temporary custody of her son and daughter. Client reported that is done with
using drugs as she feels drugs have taken over her life. Client reported that and she is ready to take back
control of her life. The client was very open about how addiction has affected her life.

Client said: My addic6on
started when I was in
high school. I guess
started using to ?t in
with the cool kids at
school. I had a great
childhood. I did not
experience any trauma
or anything crazy. I’m
sure that I have tried
just about every drug
you can think of. You
name it 10/10 I tried it.
Now that I think about it
I’m very very lucky to
alive today. My ?rst
drug of choice is meth,
and my second drug of
choice is heroin and

IV. IMPRESSIONS/ASSESSMENT: 1) What did you observe throughout the session -- behavior and
aDect; 2) was the behavior/aDect appropriate, explain; 3) how does this behavior/aDect ?t with what
you know about the client’s past behavior/aDect; and 4) iden6fy the major themes/issues that emerged.
Based on my observation the client reported that she began using when she was 16 years old
when she was in high school.

V. USE OF PROFESSIONAL SELF: Choose two signi?cant interven6ons you made: 1) iden6fy/describe;
2) what was your impression of your eDec6veness; and 3) what would you change.
CBT would be a great interven6on for this client. The client reported that she has been dealing
with her addic6on since she began using in high school at age 16. It is very important that the
client gets medica6on to help treat her cravings. This is the CM’s ?rst session so I would have to
get feedback from her on how her an6-craving meds, therapy are working for her to see if there
is anything I would change.
Individual therapy and group therapy would be a great interven6on to use with this client.
Individual therapy with a licensed therapist will help the client get to the root of her addic6on.
Group therapy will help the client to see that she is not alone and that there other individuals
with the same or similar stories like hers and that it is possible her to beat her addic6on and her
life back like she wants. This is the client’s ?rst session so I would have to see how her individual
and group sessions go to see what I would change.
VI. PLANS: (Brief statement of your plans for the next session, long range goals, short range goals that
are relevant for this client.)
My plans for the next session are to see if the client has started any an6-craving meds and if so, how are
they working for her, discuss with the client what coping strategies is she using. Short terms goals for
this client is to help her iden6fy eDec6ve coping methods., and long-term goals with this client is for her
to maintain her sobriety.
VII. ISSUES, QUESTIONS OR PROBLEMS: (To explore in supervisory sessions.) Areas to explore in your
supervisory conference: include issues of diversity, value dilemmas, counter-transference etc.
Dilemmas: I had diJculty with this session because the client was s6ll in ac6ve detox and she had
diJculty staying awake.
EXPLANATION OF PROCESS RECORDING SECTIONS
Interview Content: Begin this sec6on with a short descrip6on of where and when (the date and 6me)
the interview took place, (e.g. standing in a hallway, mee6ng in an oJce, etc.), who was present, and its
purpose. For purposes of con?den6ality, change the name of your client or use ini6als only.
Select a 10-15 minute exchange and record word for word what happened during the interview
including verbal and non-verbal content, which may include silence, gestures, and other details that
occurred during the interview. The goal is to write the exchange as closely as possible to the actual
interview.
Skills Used: Iden6fy the social work skills u6lized throughout the interview. Iden6fy the skills you
learned from your social work courses. Include the theory behind your prac6ce and why you chose a
par6cular technique, and what knowledge of human behavior helps you to understand this interac6on.
This sec6on will raise your awareness of the techniques you are using in your prac6ce.
Your Thoughts/Feelings/ Reac.ons: Record how you were feeling as the interac6on was taking place.
Before you write this sec6on, take a few moments to read through your exchange sec6on by sec6on and
then reLect back to how you felt during that 6me. Describe your subjec6ve reac6ons (thoughts, feelings,
sensa6ons) to your own words during this exchange. How did you respond to your own words or those
of your client? Analyze why you think your interac6on with the client unfolded in the manner it did.

Client Feelings A/ect: Interpret the interview. Include detail about the tone and climate of the seMng,
your ini6al impressions of aMtudes and feelings of the client, what aMtudes and feelings you brought to
the interview, what signi?cant changes in the client occurred during the interview in terms of
appearance, gestures, and posture?
Field Supervisor Comments: Supervisor provides cri6cal feedback in wri6ng to the student, and
discusses their review of the process recording with the student during supervision. The focus of the
Field Supervisor’s analysis should be wriNen in clear language so that the student understands the
educa6onal value of the feedback, including references to social work knowledge, values and skills.
