NUR 425 – Population-Focused Nursing for Diverse
Communities
(Prerequisite: NUR 300: Transition to Baccalaureate Nursing Practice, NUR 310: Quality
and Safety in Healthcare)
COURSE DESCRIPTION
Explores population-focused nursing, community/public health concepts, and population health trends on
the continuum of individuals, families, aggregates, and communities. The course provides theoretical
content essential to the assessment, planning, implementation, and evaluation of communities and
aggregates, and examines the effects of contemporary issues and health policy on the public’s health. In
experiential learning, students will conduct a community assessment and use evidence-based research to
develop a plan designed to improve or promote the health of a select population.

1. Describe the nature, philosophy, and scope of community health nursing.
2. Develop understanding of community and population health needs based on the core functions of

5. Apply the concepts of health promotion and disease prevention to communities and populations.
6. Apply knowledge of epidemiology to assess and diagnose community health status.
7. Explore issues within an identified population, in collaboration with appropriate leaders, to
improve the health of individuals and populations.
8. Analyze the public/community health nurse’s role in promoting health equity, access to care, cost
containment, and reducing healthcare disparities.
9. Examine the effect of interactions between policy and economics on the delivery of health
services.
10. Describe cultural competency in public/community nursing settings.
11. Identify the impact of contemporary issues on the health of the public.
12. Analyze population health trends and risks using epidemiological data at the local, regional, state,
and global levels.
13. Develop an appropriate plan of care for a population, using relevant data and best practice.
14. Integrate evidence-based practice to guide outreach, screening, teaching, and implementation of
an intervention to diverse individuals and communities.
15. Propose and present a plan of care that will address the needs of the assessed population.
16. Describe exemplary public health nursing practice as defined by professional standards and
organizations.
17. Integrate ethical standards into public/community health nursing practice.
18. Analyze the public/community health nurse’s role in promoting health equity, access to care, cost
containment, and reducing healthcare disparities.
