In the present case study, the events that led up to patient misidentification is during
the Osteoporosis surgery procedure. This procedure was not exactly intended to schedule for
those patients. This event started because of the admission of two patients who have the
same record numbers, same names as well as same age group. In the present case, the two
patients’ names were Jane Morris and Joan Morrison, who are at the age of 35. Here, Joan
Morrison was misidentified and mistaken for Jane Morris. The patient Joan Morrison was
transported to the surgery room for the osteoporosis surgery procedure. There were many
errors that were saying the patient was misidentified and the wrong person. Here, the staff
have taken the consent of the patent by asking whether the patient is ready or not for the
procedure. However, they missed the important protocol to identify the patient and follow
the proper protocols. In this case, the physical safeguards have been ignored in the hospital
due to the systematic faulty information exchange among the nurses, doctors, and other staff.
While measuring the chronology of events, the role of data collection and data
analysis is very significant. The collection and analysis of data help to gather information
and measurement of the patient to advance treatment strategies, customise treatment plans,
upgrade well-being results and improve correspondence among the patients and specialists.
The data collection process is basically a process in which all the information is estimated
and assembled through additional study, exploration and investigation. Here, it is important
to maintain the exact information assortment so that the probability of errors can be reduced.
The right data collection measures are necessary for the healthcare settings as it
ensures that the assembled information is precise and characterised. These data collection
measures are helpful to keep research integrity and ensure the quality of assurance
(Simplilearn, 2022). According to author Nolan, there are three tasks that need to be
attended by the designers of the care system that includes preventing errors, making errors
visible and mitigating the effects of errors (Nolan, 2000). From my perspective, important

steps could be taken by implementing active communication among the staff, which can
help them to identify the patient. Here, the probability of occurring errors can be reduced by
following the exact information assortment.
A human can make many mistakes or errors as the ordinary spectrum of life. If the
protocols are not followed properly in the healthcare sector, then errors will occur. In the
present case, precautions can be taken by following quality health services standards and
reducing human errors.
