Top three concerning issues that I see with this client is, the struggles that she is facing
with her sister’s death, the severe addiction issues that she is facing at the present time, and
lastly, the dependency issues that she is having with her daughter.
At an early age the client had a traumatizing moment when she viewed her sister being
struck by a car resulting in her death. She is harboring guilt because she was distracted when the
car approached the crosswalk, and she feels this is due to her own fault because of her lack of
attentiveness. The client refused to discuss the accident and due to that she is in heavy addiction
to alcohol.
Another issue at hand is her addiction to alcohol that started in the teenage years while in
high school. She stated that the first drink was at a friend's sleepover, they found her parents
alcohol and the first drink made her euphoric and showed her the happiness that she has not
experienced in a very long time and was caught drinking but the actions were chalked up to a
“phase” that the children are going through at the time. She was charged with DUI, and she
sincerely felt that it was not her fault. Previous drugs use she states, marijuana, and its effects
made her paranoia then later she experienced cocaine and felt signs of euphoria and contentment
and eventually had to quit due to the costs of it after she had her children. She expresses that she
would love to feel the euphoria from the cocaine again.
Lastly, I would like to address the client's issues with the dependency that she has on her
ten-year-old daughter at this current time. She expresses the depends on that she has with her
ten-year-old daughter's need to assist her with her younger children's needs and that she has an
open relationship with her oldest daughter to share information that is inappropriate with the
“mental and physical” dependecy she has with her daughter.
The client states that she would be lost without her oldest daughter's assistance with the
children and her mental state and that my fear is that the child’s mental well-being is at risk.
