The position of Community Health Worker that was posted by Source wise. According to
their job posting, this position requires someone who would act as case manager. The
community health worker would meet with the client and would explain to them what services
are being offered. The case worker would assess the client for what services they need the
most, walk them through the enrolment process, and connect them with the service providers
they need.
Certain needs might require only motivational intervention. For instance, an elderly who had
stopped going to church because they are no longer able to drive may feel isolation and
separation from their community. c With the company's services, a case manager can help them
connect to transportation services that can take them where they need to go.
Other needs may require a crisis intervention and would need to be escalated. An example
would be if an elderly is malnourished since they are unable to go to the store or cook for
themselves. They can immediately be enrolled in the Meals on Wheels program to help them
with their nutritional needs. They may even be given other options if staying in their homes
without any assistance is no no longer safe.
The organization I have chosen for my final project, Arkansas Department of Human Services.
As a Licensed Social Worker my duties are. "The Licensed Certified Social Worker is
responsible for supervising social work and counselling activities, providing technical
assistance and developing program plans. This position is governed by state and federal laws
and agency/institution policy". c
The function of the job:
"Performs a lead role in coordinating predetermined work assignments of co-workers and/or
subordinate employees, by communicating prioritization of activities and project deadlines.
Provides technical assistance to staff and clients, by offering consultation concerning specific
requests, explaining laws and regulations, and monitoring certification standards. Develops
program plans by determining goals and objectives, assessing programs, and making
recommendations for policy and procedure changes in conjunction with other staff. Conducts
individual group counselling sessions to increase client self-esteem and to aid client's social,
emotional, psychological, and physical wellbeing. Refers clients to other sources of help,
monitors client's progress, writes release summaries, and notifies appropriate personnel.
Testifies in court concerning client's compliance with court order and psychiatric stability.
Prepares and maintains reports, including logs of daily progress of patients, quality assurance
records, documentation of sessions, and case work records. Performs other duties as
assigned". The opportunity to join the United Way of Suwannee Valley Organization's client-
facing team as a "Project Coordinator" enables me to fulfill my passion while sanctifying and
impacting the lives of others. According to United Way of Suwannee Valley's Organization,
an integral part of the Project Coordinator position is recruiting and managing ReadingPals
volunteers and sites, a United Way initiative promoting early literacy, and overseeing various
other projects. There is a strong emphasis on establishing a recruitment initiative that will
result in volunteers serving children at kindergarten sites in Columbia, Hamilton, and
Suwannee Counties. In cooperation with United Way affinity groups (such as Women United)
and community partners – Columbia County Public Schools, Hamilton County Public
Schools, Suwannee County Public Schools, and several United Way committees, the
coordinator facilitates these efforts. In compliance with United Way requirements and
assignments, performing related tasks and all interactions with constituents should
demonstrate adherence to the United Way Code of Conduct (United Way of Suwannee Valley,
2022).
A Project coordinator's position fulfills various roles and responsibilities and is the primary
source of communicating information and contact between teams, managers, and
stakeholders. In addition to managing, meditating, troubleshooting, and reporting, they must
be mediators. To illustrate, the project manager defines the project from its inception to
completion. In contrast, project coordinators ensure that the project development cycle runs
smoothly. You often use written and oral communication skills daily to convey your ideas and
offer guidance, i.e., (writing emails, creating reports, and speaking) with other project
professionals. As a project coordinator, organization skills are imperative. To collect data and
properly file required documents related to a project, it is necessary to maintain a great deal of
information, including deadlines, requirements, schedules, and budgets. In budgeting and
project coordination, aim to reduce expenses while maximizing quality but streamlining
projects to save money. In addition, this role is vital to delegating tasks based on employee
skills and resolving issues as they arise (Indeed Editorial Team, 2022).
Likewise, project coordinators can evaluate the quality of work and suggest ways to improve
systems or policies to fit the needs of a team and benefit the organization's overall goal to
provide excellence in service to others. For example, the project coordinator requires the team
to participate (in some way) through the planning process, commit to the project plan, fulfill
assignments, monitor the project, and provide updates to Project Sponsors and stakeholders.
Teamwork is essential to a successful project, which is what the Project Coordinator ensures
