• What is your clinical problem of interest?
Using a standardized approach to screening and recommendation for housing to improve
individual wellbeing of homeless young adult males with exacerbated mental health conditions
• What is the current research evidence that supports the intervention that corrects the
problem?

• What is the Theory and/or EBP model you selected to guide your project?
The Rosswurm and Larrabee (1999) model for change to evidence-based practice will be
the theoretical framework of choice. The evidence-based model takes practitioners through the
whole process of converting clinical theory to evidence-based practice. The model, when put in
practice starts with an assessment of the need for change and ends with incorporating an
evidence-based procedure.
During the second step of the Rosswurm and Larrabee (1999) model for change, the
shareholders express their problem in a standardized language to discover relevant research and
tie the problem to results. The stakeholders should decide the intervention, which included a
comprehensive morale-boosting and training program for staff nurse attending to homeless
patients with mental health issues. Thorough educational activities for both management and
nursing personnel are beneficial in reducing mental health difficulties among the homeless
population.
• What were your primary project implementation steps?
My project has 7 implementation steps that has Consenting process, data collection,
Recommendation for housing, Follow up data (post-implementation data) collection, Data
protection after week 10 follow up data collection, Evaluation of project data, Involvement of
project team and Project closure. (See worksheet for elaborate steps).
• What were your outcomes and how did you measure whether or not your project was
effective in addressing the problem?
Individual wellbeing before and after standardization of screening and recommendation for
housing. Anticipated increase in well-being once participant has resided in temporary housing.
Corresponding decrease in exacerbation of mental health conditions after implementation of the
intervention/participant has resided in temporary housing.
• What are some opportunities for nursing leadership because of your project?
My Evidence based project offers great opportunities for nursing leadership. For
instance, I encourage open-communication strategy that enhance nurse input. Regardless of
title or position, all the health facility employees will be encouraged to contribute and
participate in strategic decision-making. Adopting a habit of daily stand-up meetings amongst
team members and bi-weekly stand-up meetings between teams is an intelligent approach to
start.
• What were the opportunities for interprofessional collaboration?
I anticipate that communication with housing and shelter agencies during my evidence-based
project will be smooth. I have realized that to boost interprofessional collaboration, nurses,
doctors, and executives of health care organizations must communicate consistently. Leaders
in health care should learn to communicate in various circumstances and various ways, and
communication should lead to respect and improved interprofessional collaboration.
• What technologies did you use for your project implementation?
Technology is key in ensuring the success of my EBP. IT is essential in the data collection,
manipulation or analysis and dissemination of results. Data can be collected via online means
or entered into a database after manual collection. In addition, using data analysis programs
such as Excel and SPSS, pre and post data can be analyzed and compared for significance.
Dissemination of data can take the form of online journals that can be used to publish results
of an EBP project building on general knowledge in the field of medicine (Melnyk et al.,
2012). All the uses of IT in EBP highlight its salience in the area and its continued application
can be projected into the future.
• What were the policy implications of your project? (Can be any or all levels---local, state,
national, or international)
The findings of my EBP project will significantly influence state level intervention through
enacting favorable laws that support and offer dignified shelter to homeless individuals. Policies
that enhance coordination of services for people experiencing homelessness, both within the
state, as well as with Federal partners who provide housing and complementary service
programs, is a critical component of achieving the goal of preventing and ending homelessness.
• What are your plans to disseminate your project outcomes?
My plans to disseminate my project outcomes include but not limited to;
Publishing project findings in national journals and statewide publications
Presenting at national conferences and meetings of professional associations
Presenting program results to local community groups and other local stakeholders
Creating and distributing program materials, such as flyers, guides, pamphlets and DVDs
Sharing information through social media or on an organization's website
Disseminating information on an organization's website
Discussing project activities on the local radio
