Child maltreatment and abuse is a rising public health crisis around the globe.
Approximately 1 billing children 18 and under experience maltreatment or violence each
year. Children that endure maltreatment and/or abuse are more likely to experience
negative outcomes. Some of the negative outcomes can include depression, diminished
physical health, life-long health effects, and/or early death.

More than 10% of children in the United States are affected by child abuse. It is an
epidemic that occurs in all socioeconomic classes. Prevention and early detection of
abuse is important, and pediatricians play a vital role in detecting signs and reporting
suspected abuse. Early detection and preventative measures can be a lifesaving and/or a
life-changing step when dealing with child abuse and neglect.

Over the years, federal, state, and local governments have formed programs that aim to
prevent, detect, and investigate child abuse and neglect in the United States. Many times,
child abuse and/or neglect is a result of the child being mistreated by a family member or

someone within the home. This article discusses important programs such as child
protective services (CPS), and the foster care system.

Emotional abuse and neglect in childhood are surprisingly the least studied forms of
maltreatment in children because of the difficulty in detection. However, child abuse is
found to be knowingly associated with mental health problems, as well as negative
cognitive, emotional, social, and behavioral consequences. Sexual abuse in children has
been associated with negative impacts such as self-injury, schizophrenia, psychosis, and
anxiety. In another study it is shown that physical abuse in children can leave lasting
negative effects including eating disorders, suicidality, substance abuse, anxiety, and
depression.

Abuse is three to five times more likely to occur in children with disabilities than peers
that do not face these challenges. Having a disability increases the risk of physical and
emotional mistreatment, neglect, sexual abuse, and development. Children with
disabilities can be at a disadvantage socially, because of an impairment, and
communication difficulties just to list a few.

This study focused on the connection between childhood trauma and abuse and major
depressive disorder later in life. The study shows abnormal brain connectivity in
individuals who faced abuse and/or neglect as a child. The individuals in this study were
not chosen based on their childhood trauma and testing and imaging performed was done
so years after any trauma had taken place.

Children in domestic violence situations have been linked to have long-lasting severe
health effects. This can include children who were exposed to physical, mental, and/or
emotional violence and abuse. Not only are children that endure abuse at risk for
developing negative effects, but those that have witnessed or been exposed to
maltreatment can be affected as well. This study explains the negative outcomes for those
who are directly impacted and/or individuals that are affected as an indirect result of
abuse.

The Coronavirus (COVID-19) pandemic placed many changes on the daily life of many
individuals in the United States and around the world. Many of these changes put
additional stress on parents that increased the parental burnout rates. Over the course of
the pandemic schools were closed, wages were reduced or lost, and resources were
limited causing increased stress on parents in everyday life. An increase in parental
burnout is likely to result in increased child abuse and neglect.

epidemiology, systemic factors, prevention, and recommendations. Types of abuse, risk
factors, and health and economic burdens are explained in depth. Most importantly there
are recommendations and evidence-based practices that are given to assist in the
prevention of child abuse and maltreatment.

Over the last decade there have been many improvements in public health such as
prevention, and treatment. These improvements and focus have allowed physicians,
nurses, and health care workers to assess , care for, and treat children that are victims of
abuse. This also allows health care workers to take the steps needed to prevent future
injury or even possible death.
