treatment plan.

ACTIVITIES:
The medical professionals

treatment plan. There will be
scheduled counselling
sessions, provider visits to
discuss options and address
any concerns that might rise

program.

with counsellors and medical
providers to discuss progress

will be evaluated based on the

Patients may be afraid of

CONTEXT or CONDITIONS:
Educational flyers and brochures will be distributed to patients to the Kirk Army Health Clinic, and to select participants based on
their current smoking status. Initial symposium to will be held to introduce the program and discuss terms and conditions involved.
There will be mandatory monthly counselling session, and visits with medical professionals to address any concerns and monitor
progress. Chantix will be provided to participants, covered by individual’s health insurance. Participants should be willing to quit
smoking.
