Present level of educational performance: Subtests from the Brigance Reading
Inventory indicate that Jackson's current reading instructional level is at a 3rd grade level.
This is indicative of affecting his success and progress with the 5th grade curriculum.
Brigance Reading Inventory also states that Jackson has difficulty decoding unfamiliar
words. He is able to substitute consonant sounds and long/short vowel sounds within
simple words, but struggles with multisyllabic words. Jackson is able to identify prefixes,
root words, and some suffixes. Shown from assessments and observations, Jacksons
current reading level is grade 3. Jackson shows difficulty with comprehension, only
answering 5/8 comprehension questions correctly. Jackson can comprehend and listen to
the information in his history and science textbooks, especially when he reads them
independently. Jackson is able to spell during tests, but oftentimes those words are found
misspelled in his work. In his work he has spelling and punctuation mistakes consistently.
He also struggles with paragraph structure. His accommodations preferred are a word
processor, spell checking tools, work prediction programs, and any chance to respond
verbally/orally has been helpful to understand his thoughts more in depth.
Annual Goal: After reading a 4th grade passage and answering comprehension
questions, Jackson will be able to point out in text answers three out of 5 times with 70%
accuracy by the end of the academic year.
Short-term objective: Jackson will read a 4th grade level paragraph and be able to answer
comprehension questions from it with 70% accuracy

Present level of educational performance: Jackson is able to spell words during
tests/quizzes, but the same words are found misspelled throughout his work. There are
often consistent mistakes in his spelling, punctuation, and paragraph structure. Jacksons
preferred tools to help him are word processors, spell checking tools, word prediction
programs, and opportunities to respond verbally/orally. Jackson will be able to spell
words, and put them into sentences by the end of the academic year.
Annual Goal: When given a list of 4th grade spelling words, Jackson will be able to
spell them correctly and write them in four out of five trials at 90% accuracy by the end
of the academic year.
Short-term objective: Jackson will be able to spell 5 4th grade level vocabulary words
with 90% accuracy within two months
Short-term objective: Jackson will be able to spell 10 4th grade level vocabulary words
with 90% accuracy within 4 months

sounds. Jackson can also substitute long/short vowel sounds into simple words, but has
difficulties with multisyllabic words. Jackson cannot identify root words, prefixes, and
some suffixes. Jackson will be able to identify new prefixes, root words, and suffixes by
the end of the academic year.
Annual Goal: When instructed orally, Jackson will be able to write five sentences
correctly to include spelling, capitalization, and punctuation in four out of five trials with
90% accuracy by the end of the academic year.

Present level of educational performance: Jacksons current reading level is at grade 3.
Jackson according to Brigance Reading Inventory. He struggles with root words, prefixes,
and some suffixes. Jackson often relies on his background knowledge to answer
comprehension questions. When answering orally, it is easier to understand his ideas and
thought process. Jackson has trouble focusing, and often struggles to finish his written
reading and writing assignments. Jackson prefers to use word processors, spell checking
tools, and word prediction tools whenever possible. By the end of the academic year,
Jackson will be able to recall several details including main ideas and supporting details
of a 4th grade reading passage.
Annual Goal: After reading a 4th grade passage, Jackson will be able to determine the
main idea and supporting details of the reading 4 out of 5 times with 80% accuracy by the
end of the academic year.
Short-term objective: Jackson will be able to answer 5 questions correctly from a 4th
grade reading passage with 80% accuracy within 2 months
