Presenting Problem
Joanne, a 45-year-old female, came to my office to express her appreciation towards me
and how important I am to her. Upon further questioning, she admitted to me that she possessed
a gun in her car. I made attempts to illicit more information and she refused to answer my
questions only stating she “just wants to go for a drive.” She suffers from long-standing
depression and has attempted suicide three times. She is under the care of a psychiatrist but is
noncompliant with the medications prescribed to her.
Precipitating Event
Joanne suffers from long-standing depression and guilt. She has three past suicide
attempts where twice she overdosed and once she cut her wrists. She is under the care of a
psychiatrist to treat her depression but is noncompliant with the medications prescribed to her.
She is fairly isolated. She does not date and is a single mother of a 13-year-old daughter from a
marriage that ended 10 years ago. For the last 7 years she has been estranged from her sister over
guilt of having an affair with her sister’s husband, who continues to pursue her. She has no
hobbies and only finds satisfaction in her highly stressful job.
