REFERRAL QUESTION/REASON FOR TESTING:
Mr. Ibaka was referred to Krystal Kounseling by The Mission Board for an evaluation to see if
he would be a good candidate for participating in a summer mission trip. This trip is in the very
challenging environment of Mali, which could potentially pose a threat to Mr. Ibaka and his
peers, considering his history of antisocial behaviors stemming from his hyperacusis disorder.
The referring source reports that Mr. Ibaka frequently has to be monitored to prevent him from
getting separated from his group while out in public, in an attempt to find comfort from noises
that may be disturbing to him. In addition, the referring source states that they are aware of the
potential threat this trip may pose for Mr. Ibaka, but they believe the opportunity of this trip
could also help Mr. Ibaka strengthen his interpersonal skills since none of his diagnoses are life
threatening.

EXAMINEE BACKGROUND:
Mr. Ibaka is a 25 year old Caucasian male, who presents short and stocky at 5’6”, roughly
weighing 225 pounds. Not working, nor attending school, he resides with his mother in her
home, his wife, and his seven year old daughter. Mr. Ibaka indicates that everyone in the
household gets along for the most part, but has their share of family drama on occasion. Mr.
Ibaka presents as a free spirit, who is very caring and thoughtful. During the initial interview, he
wore loose-fitting sweatpants with an even bigger t-shirt, appearing to be well relaxed. Balding
in the middle of his head, Mr. Ibaka presented with shoulder length brown hair that was slicked
back into a thin ponytail. Mr. Ibaka reports that he is typically quiet, and keeps to himself until
he feels that he can open up. He appeared to be in a cooperative mood, which enables him to
establish a good rapport with those who he is unfamiliar with. Mr. Ibaka did not present with
suicidal and/or homicidal ideation during the interview. No risk for violence was detected.

The IPIP-NEO estimates the individual's level on each of the five broad personality domains of
the Five-Factor Model. The description of each one of the five broad domains is followed by a
more detailed description of personality according to the six subdomains that comprise each
domain, totaling thirty subdomains. Personality traits describe, relative to other people, the
frequency or intensity of a person's feelings, thoughts, or behaviors. This assessment classifies
you as low, average, or high in a trait according to whether your score is approximately in the
lowest 30%, middle 40%, or highest 30% of scores obtained by people of your sex and roughly
your age. Thus, scores below 30 are considered low, scores ranging between 30 and 70 are
considered average, and scores above 70 are considered high.

Jung Personality Test
The Jung Personality Test is an assessment that breaks down the 16 different personality types by
a four letter acronym, where people can be characterized by their preference of general attitude
with extraverted versus introverted, their preference of one of the two functions of perception
with sensing versus intuition, their preference of one of the two functions of judging with
thinking versus feeling, and a last dichotomy influencing personality type with judging versus
perceiving. The first criterion, Extraversion – Introversion, signifies the source and direction of a
person’s energy expression. The second criterion, Sensing – Intuition, represents the method by
which someone perceives information. The third criterion, Thinking – Feeling, represents how a
person processes information. Lastly, the fourth criterion, Judging – Perceiving, reflects how a
person implements the information he or she has processed.

Beck Depression Inventory (BDI)
The Beck Depression Inventory is a 21-item, self-report rating inventory that measures
characteristic attitudes and symptoms of depression. Internal consistency for the BDI ranges
from .73 to .92 with a mean of .86. Levels of depression scores range from as low as zero to as
high as sixty-three with a score ranging between 1-10 indicating that these ups and downs are
considered normal, 11-16 indicating a mild mood disturbance, 17-20 indicating borderline
clinical depression, 21-30 indicating moderate depression, 31-40 indicating severe depression,
and scores over 40 indicating extreme depression. In this assessment, Mr. Ibaka scored a 29,
reflecting moderate levels of depression.
Beck Anxiety Inventory (BAI)
The Beck Anxiety Inventory is a 21-item scale developed to assess the severity of anxiety
symptoms. Those taking the test are asked to rate each item on a 4-point scale ranging from zero
to three. Items are summed to obtain total scores of anxiety levels ranging from zero to sixty-
three, where zero to twenty-one (0-21) indicates very low levels of anxiety, twenty-two to thirty-
five (22-35) indicates moderate levels of anxiety, and anything over thirty-six (36) indicates
severe levels of anxiety that is potentially a cause for concern. In this assessment, Mr. Ibaka
scored a thirty-two (32), reflecting moderate levels of anxiety.

CONCLUSIONS AND RECOMMENDATIONS:
Mr. Ibaka is a 25 year old Caucasian male who was referred for an evaluation as a potential
candidate participating in a mission trip to the challenging country of Mali. He is married with
one child, and currently does not work or attend school.
Mr. Ibaka was provided with a battery of psychological assessments, which included the
Examinee Biography, IPIP-NEO, Jung Personality Test, Beck Depression Inventory, and the
Beck Anxiety Inventory.
Based on the results from the assessments, this examiner has come to the decision that Mr. Ibaka
is an acceptable candidate for participating in the mission trip to Mali for the following reasons:
a.) Mr. Ibaka demonstrates being down-to-earth, practical, and conservative, which
will aid him in his rapport building efforts while becoming acclimated to the Mali
residents and their environment.
b.) Mr. Ibaka is likely to provide empathy to the less fortunate Mali population by
exemplifying his concern for their needs as a sensitive and emotional person.
