This Mental Status Exam contains gathered information concerning Raven Hayes. This
exam has been prepared to assist in determining if Ms. Hayes is an appropriate candidate for the
RA position at Liberty University. The information contained in this report is the clinician’s
observation of Raven during the Initial Interview.
Appearance and Behavior
Raven H. is single female who identifies as African American. Raven’s chronicle age is
19 years old and appears to about 5’7 and weighs 160 pounds. At the time of the interview,
Raven appeared to have a smooth complexion with no other identifying marks, such as scares,
moles or birthmarks on her face. Raven appears to be healthy and has a muscular build. Her hair,
nails and overall appearance was neat and well groomed. Raven’s attire was seasonally
appropriate and professional.
Raven maintained good eye contact during the meeting and was direct when answering
questions. Raven appeared to be engaged, alert and expressive during the meeting. Raven often
smiled and laughed during appropriate times. Raven appears to be good historian and could
recall information easily. No atypical physical characteristics were noted and there appeared to
be no signs of drug or alcohol intoxication.
Sensorium and Mental Ability
During the interview, Raven appeared to be open and friendly when answering questions.
Raven was candid and used large hand gestures whenever she recalled funny or excitable
memories. Due to a college GPA of 4.0, as reported by Raven, Raven appears to have an average
to above average IQ. Raven has not been formally assessed.
Orientation, Alertness, and Thought Processes

Raven appeared to be alert and oriented x 4. Raven’s speech was at a normal rate and
volume and did not appear to have any psychomotor agitation or pressured speech. Raven made
eye contact at appropriate times and appeared to have no problems expressing herself through
speech. Raven did not display issues concerning comprehension and appeared to understand all
questions with ease.
Raven appeared to be a strong historian and could recall dates, times and other specific
information when recalling past and current events. During the interview, Raven was asked to
recall three words (house, cat, blue) and was asked to repeat the words back to writer 5 minutes
later. Raven recalled all three words with ease.
During the interview, Raven denied hallucinations and delusions. Raven displayed a
logical thought process and all thought content was in normal limits. There was no evidence of
compulsions or obsessions noted during the interview.
Mood and Affect
Raven was easily able to establish rapport and was open and friendly during the
interview. Her facial expressions were appropriate for the conversation and Raven has a low risk
of violent behavior and displayed acceptable impulse control. Raven denied a history of currently
experiencing homicidal or suicidal thoughts or behaviors.
