Reason for Referral
Mr. Tristan Harris has been referred by Dr. Ford because he is a candidate to be a Residential
Assistant (RA) at Liberty University. Mr. Harris is coming in for an evaluation to determine how
effectively he can communicate and help others in adherence to rules and regulations of
dormitory life.
Current Situation/Functioning
Mr. Harris enjoys starting his mornings with gospel and fruits. He is a full time student
and only has classes Monday through Thursday. He completes his assignments ahead of time.
His wife goes to Liberty as well, she is getting her doctorate in nursing. Tristan reports that
because of their busy lives, he and JaDani do not get to be intimate as much as they used to. His
anger stems from frustration and sometimes he has tantrums and will go through different phases
of anger behind closed doors.
In terms of basic ADL skills, Mr. Harris has some positive and negative skills that call for
concern. His positive aspects include ambulating, housekeeping, transportation, toileting,
continence, and house maintenance. When looking at certain aspects, he lacks hygiene and
shopping/meal preparation. Mr. Harris reported that he only spent five minutes in the shower and
