WORKSHEET 6.1A: Evaluation Planning Questionnaire

Use the filled-out Worksheet 6.1B in the book as an example to follow as you complete this questionnaire.

1. What questions will your organization’s evaluation activities seek to answer?

The evaluation activities will seek to answer the questions; Has the program achieved its goals? Were the objectives met? What are the strenghts and weakneses of the program? What can be done to improve the program?

2. What are the specific evaluation plans and time frames?

a. What kinds of data will be collected?

FBWC will collect various kinds of information for assessment. It will gather the number of sexual assault and domestic violence survivors that will have attained self-reliance since the onset of the progam. It will also collect data about the number of homeless individuals that will have got permanent residence after the implementation of the program. Lastly, the organization will collect data regarding the satisfaction levels of the participants and public concerning the program.

b. At what points?

Data will be collected within the organzation and around the community (Richmond City)

c. Using what strategies or instruments?

The organization will use both quantitative and qualitative data collection approaches. Quantitative strategy will be applied in gathering data of participants that would have attained self-dependence as well as those outside homelessness situation. Qualitative approach will be utilized in collecting the satisfaction levels of the participants and the public regarding the program. Moreover, quesionnaires, interviews, and observation will comprise the data collection methods that the enterprise will employ.

d. Using what comparison group or baseline, if any?

The comparison group will entail individuals that willl not participate in the program. Their general life will be measured against the particiants of the program. Comparing the two will allow appropriate evaluation of the program effectiveness.

3. If you intend to study a sample of participants, how will this sample be constructed?

In this case, the sample will comprise only individuals that will participate in the program. The sample will be constructed based on criteria that will be put in place in selecting the participants to benefit from the program.

4. What procedures will you use to determine whether the program was implemented as planned?

To determine the implementation of the program based on the plan, assessing the achieved goals and objectives will comprise the key guides. Comparing the actual outcomes with the set targets will also show whether the program is execuated according to plan (O'Neal-McElrath, 2013).

5. Who will conduct the evaluation?

The program manager will be in charge of the evaluation process

6. Who will receive the results?

The results of the assessment will be provided to the participants, organization, partners, and the public.

7. How are you defining success for this program or project?

Success for the program will be defined by a reduction in the number of homeless individuals in Richmond City. An increase in the number of self-reliant survivors of sexual assualt and domestic violence will also portray the success of the project.
