Week 9 Journal

Felicia is a 17-year-old female who presented to Beckett Springs mental health facility, with her mother voluntarily for “I can’t think straight anymore.” She has a history of psychosis, mood instability, and behaviors putting herself and others in harm’s way. She is noted to have difficulty with agitation, internal preoccupation, and is increasingly suspicious of others. The client endorsed difficulty with rumination and self-sabotaging thoughts, feeling that she is being poisoned. She also endorsed difficulty with sadness and anxiety symptoms. She states she is having suicidal thoughts with a plan to slash her wrists. She is noted to have increased delusions and fear with a sense of impending doom. Due to her suicidal ideation, psychotic symptoms, mood instability, and poor judgment, the client is referred for hospitalization at this time. Felicia has a history of hypothyroidism, asthma, and gastrointestinal esophageal reflux disease (GERD). She has no known allergies. Her current medication list consists of:

Albuterol rescue inhaler PRN Asthma

Geodon 20 mg BID Psychosis/Mood

Prozac 20 mg Daily Depression

DSM-5

The client’s diagnosis is major depressive disorder, severe, recurrent, with psychotic features. She presents with symptoms greater than two weeks. She reports a decrease in things she once enjoyed doing with feeling sad most of the day (American Psychiatric Association, 2013). She has recurrent thoughts of dying and now has a plan. The psychotic features are related to her delusions of voices telling her to slit her wrists (American Psychiatric Association, 2013).

Legal/Ethical Implications

With every client, the psychiatric mental health nurse practitioner (PMHNP) must ensure safety and focus on harm prevention. According to Nagy (2011), informed consent about the aspects of treatment and all services must be provided to the client. There is a need to integrate knowledge within the therapy setting with the client and the family while also maintaining confidentiality (Kocher, 2003). Setting a therapeutic alliance and treatment goals is essential while paying close attention to the right and needs of the adolescent client (Kocher, 2003).
