Description of the Organization

Change implementation embeds on the practice environment in which change is to be implemented.

In my organization there is a supportive leadership and committed health professionals and educator.

The work culture is based on competitive salaries, opportunities for career development and professional growth which empowers nurses to remain committed to providing quality care.

Information is also easily accessible and there are adequate resources for change implementation.

Nurses are engaged in decision making through shared governance.

Opportunity for change

The PICOT evaluation suggest that psychotherapy is the most effective treatment for PTSD patients.

However, in my organization, healthcare professionals and psychologists lack training in trauma-focused treatments needed to provide care to PTSD patients.

The poor health care infrastructure and limited training institutions hinder medical doctors from providing evidence-based treatments.

There is shortage of medical doctors trained to handle PTSD patients.

Doctors psychotherapy to patients without PTSD or co-morbid diagnoses such as those with current psychosis.

Stakeholders affected by the issues include;

advanced practice nurses

professional development educators

organizational director and head of operations.

Risks associated with the change implementation include limited resources, delayed implementation, and reluctance among doctors to undergo trainings.

Evidence-Based Idea for Change

Individuals with PTSD are prone to nulliparous and it is important to the relationship between mental health and treatment preferences specific to PTSD (Lancaster et al., 2016).

Medical doctors lack knowledge about specific treatment such as psychotherapies relevant in treating PTSD.

Numerous studies provide evidence that psychotherapy is effective in treating PTSD (Goodnight et al., 2019).

Evidence also shows that there will be shortage of trained health care professionals in evidence-based trauma interventions in future (Tanaka et al., 2017).

It is important to involve volunteers and non-professionals in disseminating knowledge and information about managing the psychological needs of trauma victims.

Knowledge Transfer

Knowledge creation will involve conducting research about the effectiveness of psychotherapy in treating PTSD and making specific recommendations based on research findings.

In knowledge dissemination, I will partner with professional opinion leaders and the organization to disseminate knowledge to relevant stakeholders.

I will disseminate brochures to nurses and physicians.

In organizational adoption, I will involved an advanced care nurse to pilot the change in certain PTSD patients. The results of the pilot implementation will form the basis of getting the organization to adopt evidence-based practice in treating PTSD patients.

Measurable Outcomes

The number of medical doctors with knowledge about trauma-based treatments.

The percentage of patients treated using psychotherapy.

The rate of misdiagnoses for patients with current psychosis or have no memory of trauma event.

The number of patients who have recovered after psychotherapy

Summary of the Critical Appraisal

The articles contained level I and III evidence because evidence was obtained from systematic review and structured controlled trials respectively.

The researchers provided insightful information about PTSD based on evidence findings which can be used for future research.

the selection process of the study participants was well-structured, making it easy to generalize results and avoid bias.

Lessons from the Evaluation Table

Evidence based practice is guided by a patient problem that is based on accurate assessment and aligns with current professional practice.

Evidence-based research must incorporate best external evidence, reflect patients’ outcomes, and show the need for practitioner's clinical expertise

Interventions for EBP must be justified with the most valid evidence.

Healthcare providers’ knowledge and skills are vital in evidence-based practices.

The personal scope of practice is based within medical doctors’ knowledge comprising of relevant education, training, and competency to provide quality care.

There is shortage of medical doctors competent to handle trauma-based therapy.

Patients’ needs are vital in evidence-based care and they should be considered in providing best practice care.

Although random control trials , systematic reviews, and meta-analyses evidence are the most used in designing practice protocols, clinical expertise, best available evidence, and patient’s vales are the most critical determinants of evidence-based practice.

Best practice in nursing should integrate all three factors for a comprehensive evidence-based approach in providing care.
