CC: pt visited for endometrial biopsy.

HPI: new onset of Hypertension and leiomyoma. Overall good health. Just started to take Losartan

25mg PO daily for HTN by her pcp.

Allergy: PCN and sulfa antibiotics Reaction is Anaphylaxis.

ROS is all normal

V/S: 155/80 HR:83 RR:20 Temp:98.4 Wt:87.91kg

DX: N85.00 D21.9 N83.202

Plan: for N85.00, obtained consent for endometrial BX. Risks and benefits were explained.

Lab will be done ( pathology report – 500918 )

For N83.202(Lt ovarian cyst) planning for laparoscopic left ovarian cyst excision/removal. Needs

authorization from insurance company.

Next appo is 2wks for bx result.
