Presenting issues and purpose of contact:

A case manager from the MS society contacted sound options because the client and her husband are both on SSDI and the husband has been taking care of his wife but is now in need of caregivers to come help him. This is the initial assessment process.

O: Well, I have been living with this throbbing pain but I also have this open sore. I have not seen my doctor in a long time

S: I am sorry to hear that. I would like to recommend you make a doctor appointment it would help with assessing your pain and how to alleviate it. Plus we need a wound assessment

Grounded Theory reassurance encouragement

Student Self-Assessment: I started with establishing rapport. This was the first meeting hence the comfort was an important element to allow an active communication pattern and establish a strategy of communication. In addition, there was the establishment of roles earlier on to increase relations and draw boundaries that may be detrimental to intervention. through the assessment, there was consideration for learning about the patient which was a good insight into the expectation of the patent.

Plan for next Interview: Next week we will talk again to see if they have made the doctor appointment and we will be filling out an application for a cooling vest at that time.
