Process Recording
INFORMATION THAT SHOULD GO INTO PROCESS RECORDING
FOR MICRO WORK
There are various formats for completing a process recording. The following is an outline that covers the major areas we want included within a process recording. Please utilize the template that follows for completing a process recording with an individual, couple or family client(s).
1. Description/Identifying information: The social work student’s name, date of the interview and the date of submission to the field instructor should always be included. Identify the client, always remembering to disguise client name to protect confidentiality. Include the number of times this client has been seen (i.e., "Fourth contact with Mrs. S."). On a first contact include name and ages of the client(s) you have written about. If client is seen in location other then the agency say where client was seen.
2. Purpose and Goal for the interview. Briefly state the purpose of the interaction and if there are any specific goals to be achieved, the nature of the presenting issues and/or referral.
3. Verbatim Dialogue (in the table below). A word-for-word description of what happened, as well as the student can recall, should be completed. This section does not have to include a full session of dialogue but should include a portion of dialogue. The field instructor and student should discuss what portions should be included in the verbatim dialogue.
4. Assessment of the patient/client/consumer. This requires the student to describe the clients’ verbal and nonverbal reactions throughout the session. Consider everything that is occurring such as body language, facial expression, verbal outburst, etc.
5. The student's feelings and reactions to the client and to the interview (in the table below). This requires the student to put into writing unspoken thoughts and reactions s/he had during the interview e.g. "I was feeling angry at what the client was saying, not sure why I was reacting this way…”. “ I wonder what would happen if I said such-and-such.”
6. Identify skills and/or theory/ conceptual frameworks used (in the table). The student should be able to identify what skills they used in an interaction, and/or what theoretical framework came to mind as they dialogued e.g. “I used the strengths perspective “ “I used the skill of partializing”
7. A summary assessment/analysis of the student's impressions. This is a summary of the student's analytical thinking about the entire interview and/or any specific interaction the student is unsure about. Include any client action or non-verbal activity that the student may want to discuss. (See Guided Questions at the end of the template for this section A-H)
8. Future plans. The student should identify any unfinished business and/or any short/long term goals.

1). Presenting Problem: Department of Social Service (DSS) involvement due to baby skull fracture
2). Purpose of Session (why is the client being seen): Check on client and provide a pack n play for baby to sleep when allowed to return home
2). Objectives/Goals of this session: The goal was to see if there were any other services that client may need additional assistance, provide encouragement, and assist with establishing a new goal or redirecting current goal.

Client was a little more receptive after hearing why I was at her home. I could see the client begin to relax and not to be as tense. |
I felt that my continuous professionalism and showing concern helped with client being able to relax and more willing to talk with me. |

Client began expressing how stressed she was and how the DSS social worker was not communicating with her about when her daughter would return. She talked about how the social worker always talked to her in a disrespectful manner. Client did inform me that she did have her boys back in the home with her but still waiting on when her daughter would return. |
I could see the pain and frustration of feeling like a failure. Client was comfortable and willing to provide more information. When she spoke of the return of her boys, I was able to see joy in her eyes. |
Being attentive to the client and her needs I offered her more numbers for resources that would be able to further assist. I told her that I know it may be tough dealing with her case worker but she needs to remain calm and not say or do anything that would prolong her from being able to get her daughter. |

I informed client to stay encouraged and continue working on her short-term goals. I provided client with a smaller task of writing down each day what she wanted to get accomplished and focus on a smaller perspective in helping lower her stress level. |
Client stated that she never thought about breaking her task up daily. Client was open to the suggestions and even got a piece of paper and wrote down the different ideas and new goals. |
It was really good to see her open up and the ambition of wanting to accomplish more and do what she felt was right so that she could get her daughter back and continue being a good example for her kids. |

I ended meeting with client and told her that I would send her an email with more information that would be valuable to her and she could call me anytime if she needs anything before our next meeting. |
Client said she appreciated all the information I provided and would be looking for email and our next meeting or call. |
I was super excited to see that the end result was better than when we first met. It was also good to see that the information I provided was beneficial for her. |

6). Summary Assessment/Analysis of the Session
A. Identify the stage of work with client/client system (i.e., pre-engagement, engagement, assessment, intervention, evaluation). Why ? The state of work with this client is intervention/evaluation which was to provide equipment based on prior engagement and see where the client was mentally to retrieve any additional information and give more resources.
B. What did you learn from the session that adds to your understanding of the client? During this session I was able to learn that just because a child is removed from a home does not mean that what they are charged with is always on purpose.
C. What were the major themes of the session? Exploring other methods in helping determine other methods for the client to work through current difficult times.
D. What were the challenges presented during the session? During the session it was challenging to keep her encourage. She has been working so hard and doing everything that is required and still not getting the right answers from her DSS caseworker.
E. What was accomplished during the session? During this session. I accomplished verifying any additional needs of the client and her children and provided more steps in accomplishing new goals.
F. What concepts or theories or interventions did you apply? And what were the results--what worked and what didn’t given the socio-cultural context of the client's presenting problem(s) and underlying issues? I used client centered technique, which allows the client to lead the conversation while giving unconditional positive regard. I also used redirecting due to client continuously would focus back on past negative situations.
G. Explain how your interpretation of what occurred in the dialogue relates to the week’s lesson (ie. Week #2 – Engagement, Week #4 – Assessment, Week #6 – Intervention, Week #8 – Evaluation). During my dialogue with Miss Susie I had to use numerous approaches in order for her to engage in conversation. In our weeks lessons I had revert back to the different steps with hopes of having a better outcome. The interventions that took place during our meeting that I felt was more beneficial for this session. I applied Assessment, Engagement and Intervention.
H. A summary of the student’s impression: Meeting this young lady I was encouraged ashamed at the same time. She encouraged me because I do not know many that would be able to be going thru what she was facing and still complete schooling to be a phlebotomist, complete her counseling sessions and still work. The reason I said I was ashamed is because I complain about the things I am dealing with and it is always someone in a worst position than myself. However, I realize that one should never allow their struggles subject them to becoming a failure.
I. Questions for Supervision: Where there any other techniques or formats that I could have chosen to better assist this client situations.
7). Future Plans
This client obviously had obstacles from allowing her to reach the goal she was working towards. We will focus on setting smaller steps in completing her short-term goals by writing a daily list of what she wants to have completed for that day. Working with her in doing this she will be able to see accomplishments each day even if it is just one task. When seeing that she is on a steady road then we can set a long-term goal still implementing the strategies we used in reaching the short-term goals.
