Plan-Do-Study-Act is the quality improvement selected for testing a change by planning it, trying, observing the outcomes and address according to the results obtained (Spath, 2018). The main reason for selecting the model is because it helps in developing specific improvement ideas. In achieving this, the model involves four steps: plan, do, study and act (Christoff, 2018).
Based on some of the findings made in the past week in relation to the declining of the patient, the model can be used to develop the plan for the practice experience project with the first step being to identify an opportunity to improve and plan a change. In this case, the opportunity addresses the details of nursing assessment such as minor health issues of the patient that form symptoms of declining in some patients. Also, poor communication between the nurse and the patient’s close relatives is a contributing factor that can be addressed as an opportunity. A plan on improvement in respect to this will involve increasing the nursing assessment through tests and monitors for hypertension, lung sounds and breathing patterns. Strategies of communicating with the patient and care givers will be improved through the ESAS instrument enhanced with the electronic medical record-based provider so as to improve hopice care and hence improved outcomes.
The next step would be implementing the improvement plan on a small number of patients who are in the hospice program to examine if there is improvement after the enhancement of the changes (Jiao, 2017). After the test, the results will be examined to determine if the goals were achieved such as identification of changes in the nursing assessment areas. This include whether the ESAS scores they give indications of symptoms of appetite, sleep and pain. The last stage in this model system will involve using the results to incorporate the changes in the workflow establishing future quality improvement plans.
