Practicum Experience Plan
The purpose of a Practicum Experience Plan (PEP) is to provide an outline that sets forth
objectives to frame and guide me through my practicum experience. The goal of the practicum is to
provide opportunities to synthesize, integrate, and apply practical skills, knowledge, and training
learned through courses and to further gain professional experience in a professional public health
work environment. Through this practicum I am expecting to be able to observe and learn from
professionals in the field. In the following, I will formulate a PEP for this practicum experience,
discuss a nursing theory and one counseling theory that I will put into practice to guide my clinical
practice and individualized learning objectives. Furthermore, I will set a timeline to meet my set
objectives.
Nursing Theory
The Erickson’s Modeling and Remodeling Nursing Theory is a theory that I believe would
best guide me in my clinical practice. This theory’s approach in mental health, enables practitioners
to focus on each client individual needs and uniqueness through compassion and nurturing care
(Nursing Theory, 2020). This holistic nursing theory is an attempt to allow nurses to approach
situations based on the clients perspective with an awareness of and respect for the clients believes
and needs. The modeling and remodeling theory was developed to allow nurses to recognize their
clients ability to understand what has made them sick, and what will make them well (Nursing
Theory, 2020). Essentially, being on the same page with the client gives the opportunity to plan
individualized care for patients more effectively.
Counseling Theory
In nursing, specifically in mental health, the nurse-patient relationship must hold a strong
foundation grounded by trust and respect. Peplau’s Theory of Interpersonal Relations is emphasized
by data that indicates an effective therapeutic relationship is successful when the phases of

orientation, working and termination are met (Hagerty et al., 2017). The initial visit should be
guided to show courtesy and respect to the patient, become familiar while establishing therapeutic
communication. The termination phase focuses on educating the patient regarding symptom
management and recovery (Hagerty et al., 2017). When the three phases of this counseling theory
are met, treatment and compliancy are more effective and efficient. The approach of using
Erickson’s nursing theory and Peplau’s theory as guides in my clinical experience would allow me
to establish a trusting and therapeutic rapport with my clients and allow me to care and treat more
efficiently.

Part 2: Individualized Practicum Learning Objectives Objective 1: I will take initiative to practice selecting screening tools while asking for feedback from my preceptor and making adequate changes as instructed.
Planned Activities: Take initiative and select a screening tool to assess patients across the spectrum, jointly interviewed with my preceptor and allow for constructive criticism by my preceptor, apply and learn.

Objective 2: I will individually conduct a psychiatric evaluation on pediatric/adolescents patients under supervision and practice interpreting results.
Planned Activities: Apply assessment skills learned through prior practicums and course practice to perform an age-appropriate comprehensive assessment.

Formulate appropriate treatment plans that include psychopharmacology and psychotherapeutic techniques.
Objective 3: I will be capable of identifying the need to refer to another specialty provider when applicable.
Planned Activities: I will become familiar with the interdisciplinary team at my facility while perfecting my knowledge of my NP scope of practice.

Part 3: Projected Timeline/Schedule
I intend to complete 160 Practicum hours (as applicable) according to the following timeline/schedule. I also understand that I must see at least 80 patients during my practicum experience. I understand that I may not complete my practicum hours sooner than 8 weeks. I understand I may not be in the practicum setting longer than 8 hours per day unless pre-approved by my faculty.
Projected plan is to meet with Dr. Rhodes at Terros Health twice weekly for 10hours shifts on Tuesdays and Thursdays.
