Summary of strengths:

I have a strong foundation in diagnostic reasoning, particularly in developing differential diagnoses, formulating accurate diagnoses according to DSM-5-TR, and recognizing normal versus abnormal age-related physiological and psychological changes. My pharmacotherapeutic skills are also well-developed, allowing me to confidently select appropriate evidence-based guidelines for medication management. Additionally, I excel in maintaining professional boundaries and building therapeutic relationships with clients, as well as effectively collaborating with multi-disciplinary teams to enhance clinical practice. My ability to identify and address ethical and legal dilemmas, along with my proficiency in selecting, implementing, and interpreting screening instruments, further strengthens my clinical competencies. These skills enable me to make informed recommendations and referrals, contributing to comprehensive and patient-centered care.

Opportunities for growth:

While I have a solid grasp of many clinical skills, there are areas where I can further develop. I need to gain more experience in conducting psychosocial and functional assessments, as I have had limited exposure to these types of evaluations. Additionally, I could benefit from more practice in psychotherapeutic treatment planning, particularly in selecting appropriate evidence-based guidelines and applying counseling techniques tailored to individual needs. Another area for improvement is in the follow-up and documentation of pharmacotherapeutic interventions; I need to enhance my ability to evaluate patient responses to treatment and accurately document any necessary adjustments to care plans. By focusing on these areas, I aim to become a more well-rounded and effective practitioner.

Now, write three to four (3–4) possible goals and objectives for this practicum experience. Ensure that they follow the SMART Strategy, as described in the Learning Resources.

1. Goal: Improve skills in psychosocial and functional assessments.

a) Objective: Conduct at least 10 comprehensive psychosocial assessments under supervision within the first month of practicum.

b) Objective: Perform functional assessments for 5 different patients with varied diagnoses and review findings with a supervisor.

c) Objective: Seek feedback on assessment techniques from a preceptor after each session to enhance confidence and accuracy.

2. Goal: Enhance ability in psychotherapeutic treatment planning.

a) a) Objective: Establish 5 unique psychotherapeutic client care plans that meet the evidence-based practice guidelines within the first two months of the practicum.

b) b) Objective: Assist in 3 patient counseling sessions, employing counseling techniques appropriate to the patient’s developmental level under the guidance of a professional.

c) c) Objective: Discuss the care plans with the multi-disciplinary team and make changes to the plan during monthly care plan meetings.

3. Goal: Strengthen pharmacotherapeutic follow-up and documentation skills.

a) a) Objective: Assess patient response to medication in 5 patients and modify treatment in first 6 weeks of patient care.

b) b) Objective: Conduct at least 2 case presentations in pharmacotherapeutic outcomes and evaluate feedback from peers and supervisors.

c) c) Objective: Take the necessary steps to improve the organization of the records and the coverage of all aspects of the patient work-up in 90% of the records scrutinized by the preceptor.

4. Goal: Develop a deeper understanding of differential diagnoses.

a) Objective: Create and discuss differential diagnosis lists for 10 new patients with a preceptor within the first two months of practicum.

b) Objective: Incorporate DSM-5-TR criteria into diagnosis formulation for at least 5 patients, ensuring accuracy and comprehensiveness.

c) Objective: Attend and reflect on 2 workshops or seminars focused on diagnostic reasoning and differential diagnoses.
