Creating a “culture of safety and high reliability!”

The research on high reliability organizations has identified five attributes that these organizations share that keep them on the cutting edge of error reduction:

· A corporate mindfulness that is always sensitive to the fact that healthcare is a “high-risk environment”;

· An awareness that the inherent complexity in healthcare makes risk more probable;

· A preoccupation with errors – every error or near miss is an opportunity to learn and improve;

· A respect for “experts” in the system, no matter where they are in the chain of command; and

· A resilience that allows them to adapt immediately, when necessary.
