Week 1
Consider a time when you had the opportunity to become politically involved in an issue (at work or in your community). If you pursued the opportunity what were the results? If you did not, what were the reasons?
Nurses also have other roles other than ensuring that healthcare is administered correctly and that patients understand what it takes for them to achieve proper health. An important role that is not captured in the job description of nurses is advocacy (Vitale et al., 2019). As part of being leaders, nurses advocate for different parties. The advocacy can be political or institutional. By analyzing how nurses conduct advocacy, one can understand the critical role of nursing advocacy.
There are five main ways in which nurses serve as advocates. First nurses advocate for their patients. Nurses advocate for their patients by communicating the preferences of their patients, providing information that aids decision making with intent to improve patient satisfaction and health outcomes and by supporting patient’s voices concerning care and choices (Abbasinia, Ahmadi & Kazemnejad, 2020). Secondly, nurses advocate for the communities and they do so by sharing stories concerning healthcare costs with leaders in the community and also by providing expertise concerning public health. Thirdly, nurses advocate for policy change. Nurses advocate for policy change through their nursing bodies (Nsiah, Siakwa & Ninnoni, 2019). For example, through American Nurses Association, American nurses can influence policy as concerns healthcare administration.
Nurses also advocate for themselves. Similarly, to other occupation, nurses have challenging work environments. To improve their working environment, nurses have to push for reforms and they do so by advocating for themselves. Nurses advocate for themselves through their memberships on practice committees, and participation in employee forums. Lastly, nurses advocate for the nursing profession. Through rallies and involvement in practice-based demonstrations, nurses get to push for their agenda. For example, through political strikes, nurses in areas that did not have enough personal protective equipment during the start of the Covid-19 pandemic were able to get the PPE and consequentially could administer care easily and safely.
As you reflect on this week's learning activities and your role as a practice scholar, discuss the area of your political competency that needs further development.
I remember of a time that as part of the nursing fraternity we had our working hours increased. The management of the institution that I worked for demanded that nurses work four twelve-hour shifts weekly due to the increase in number of patients that the institution was attending to. Due to the changes, nurses became more fatigued and consequently were more prone to malpractice. A group of nurses with me included decided to campaign against the changes in the shift. We advocated for ourselves and as a result we were able to secure a bargain that was well-accepted by majority of the nurses in the institution. We were able to bargain for five-eight hour shifts and additional compensation. In terms of improving political competency, I need to improve on how to disseminate information to stakeholders for proper advocacy.

Unemployment is a serious social determinant issue that is affecting our society today. There are a lot of Americans who are searching for jobs currently and this is an indication of a very high level of unemployment. The social effects of unemployment range from financial stress, homelessness, poverty, poor nutrition, and crime, all of which affect the health of an individual. The longer an individual remains unemployed, the severe the effects of unemployment become. “Those who are unemployed report feelings of depression, anxiety, low self-esteem, demoralization, worry, and physical pain” (Health People.gov). Most lifestyle diseases related to stress such as high blood pressure, arthritis, stroke, anxiety, depression, and heart disease attack the poor. Both the physical and mental health are affected by situations such as lack of job security, underemployment, and workplace closure. Unemployment is the major cause of poverty and people who cannot be able to pay for better healthcare services. Health insurance, and proper sanitation. This is why there is a strong relationship between unemployment and poor health. According to Briody, Doyle, and Kelleher (2020), the health effects of unemployment are severe on children and young people who sometimes cannot attend school due to poverty and end up engaging in crime and drug and substance abuse. All these are detrimental to their health.
As a healthcare provider, I plan to use my role to ensure the public is aware of the health effects of unemployment and the importance of employment. I will provide educational programs to individual employees and the community at large which will focus on the long-term benefits of health and wellness. The goal of every healthcare provider is to ensure a healthier society where more and more people are supported by health nurturing jobs. This will allow people to earn enough to support their families’ wellbeing. To pursue this important public health and economic priority, I will request help from other stakeholders in the ministry of public health and the government. I will request all leaders at the community level to work with me in ensuring there is employment for the people.
The success of this initiative will also call for the participation of owners of large scale and small-scale businesses. They will help in the design and development of job training programs that are best suited to today’s employment requirements. They will also provide ideas on ways of maximizing job opportunities for the unemployed.
I will also be an advocate of the people and come up with policies such as the lowering of employment taxes such that there will be high labor demand. I will forge for discussions with employers and businesses to ensure a changed attitude towards employees, health, and community wellbeing. Other stakeholders who will contribute a lot to the creation of employment include the NGOs, different public health advocacies, social service workers, state and federal social benefit providers, and faith based organizations. They will help come up with different programs that may help generate jobs for the unemployed.

Have you met with the decision makers at the practicum site to determine the type of problem they are having and ways in which your project might be developed to address this problem? What was the outcome of this meeting?
I held a consultative meeting with a group of decision-makers in the practicum site to identify the problem their healthcare facility has been experiencing in the past few years. I realized that proposing my DNP project to them would be of great benefit. Through a series of discussions about healthcare improvement and quality and safe care, the decision-makers were concerned about the rising problem of medication errors in the pediatric ward. The decision-makers in the practicum site have realized through the facility’s records that pediatric medication errors have turned prevalent and is causing harm to children and families. Based on strategies they have been implementing over the years to reduce medication errors, it was concluded that the harm caused to pediatrics is more severe than adults because their immune has not developed fully compared to adults (Salar, Kiani & Rezaee, 2020). By engaging in a consultative meeting, I presented my evidence-based facts that it is possible to prevent medication errors by implementing my DNP project. By controlling and preventing pediatric medication errors, the facility will create a positive reputation for providing quality and safe care by pediatric nurses in the region.
Please share your proposed PICOT question in question format. Is this PICOT question likely to change in the future?
My DNP project is supported with evidence-based practice in the form of a PICOT question to guide practitioners and decision-makers to use solution-based strategies in developing interventions that will reduce pediatric medication (Beretta, 2021). Pediatric prescription error is an area that has attracted attention from researchers. As a DNP, I want to explore this field to realize the detrimental outcomes that affect paediatrics under treatment. Decision-makers will use the PICOT question to design and implement interventions that will mitigate increasing rates of prescription mistakes and the adverse effects experienced by providers and patients. My PICOT question is “Does implementation of in-service training on dosage calculations for nurses working in pediatric units help in reducing cases of medication errors as compared to no in-service training?” the picot is supported with previous studies that generate high-quality evidence, and it will not likely change in the future. Furthermore, according to Dhond et al. (2021), an evidenced-supported research question is said to have passed the review test, and findings from diverse sources are affirmed to be reliable and credible. Also, my DNP PICOT project has been practised in other facilities, and through in-service training for the nurse of dosage calculations, it has reduced prescription errors resulting in improved patient safety.
Describe the translation science model you will be using or are considering using as a part of your project.
I was asked if my DNP solution-based project could be translated with a scientific proved model. My response was based on evidence because translation doe involves translating research, identifying a specific issue within a healthcare facility, and translating available knowledge to bring about practice change (Raghavan, Bhardwaj & Rani, 2020). Therefore, I will use the simulation translation method when teaching pediatric nurses about in-service training on dosage calculations supported with evidence-based practice to break down the challenges that arise with mixed messages in communication. In addition, the simulation method will engage extended multidisciplinary stakeholders because implementing change within an organization requires a collaborative effort to empirically clarify simulation goals and outcomes during feedback sessions to enhance the understanding of all levels of the proposed PICOT intervention (Luke et al., 2018).
What is the current priority you have for the development of your project and do you believe you have support for this project from the practicum site decision makers?
Lastly, my current priority is to train pediatric nurses to perform the in-service calculation of dosages and record the results in a computerized decision-support system for the next nurse on shift not to make mistakes. Based on the needs of the practicum site decision-makers, I believe they will support my project because it will solve a problem that is putting patients at risk of developing other complications, longer stays in the facility, increased cost of running the facility’s affairs (Salar, Kiani & Rezaee, 2020). Their goal is to have a positive translation effect on practice by improving patient safety and reducing medication errors.

The selected Twitter feed is from the World Health Organization (WHO) that was posted on April 7, 2021. The initiative is the COVAX initiative that was founded by the WHO and its partners to ensure that there is equitable global access to the COVID-19 vaccine. The COVAX is co-led by the Coalition for the Epidemic Preparedness Innovations (CEPI), GAVI, and the WHO with UNICEF being the delivery partner as PAHO as the procurement agent in the Americas. The World is currently experiencing a pandemic of COVID-19, it there is a need to ensure that the population is protected (Abazie et al., 2021). It helps in keeping the population from getting severe illness even if they do not have COVID-19.
It helps in ensuring that people around are protected especially those who are at increased risk for serious illness from COVID-19. Since the foundation of this initiative, millions of diseases of the vaccine are currently being distributed to approximately 100 participating nations in the entire world. The COVAX initiative has led to the acceleration of the development and manufacturing of COVID-19 vaccines (Abazie et al., 2021). The WHO reports that the initiative has guaranteed fair and equitable access to the vaccine to every nation.

One of the major approaches towards the fight against the pandemic is the development of a vaccine that can ensure that the safety of the public is improved. The WHO together with its associated partners has done a great job towards ensuring that initiatives aimed at the development and manufacturing of vaccines against COVID-19 are made successful. Through this tweet, it confirms the commitment of the WHO to ensure that there is development o0f many vaccines through its COVAX initiative (Abazie et al., 2021). It aims at ensuring that there is speedy development and manufacturing of the COVID-19 vaccines. It also guarantees fair and equitable access for every nation.
The efforts made by the WHO and its partners in coming up with these foundations have led to positive gains as the number of individuals who have been vaccinated against COVID-19 is increasing. This has led to an increase in the confidence amongst the members of the public concerning the approaches undertaken to reduce the risk of contracting the disease. However, several issues surround the development of these vaccines. For example, the safety against COVID-19 after receiving the jab and the potential side effects. There are more worries about the safety of the public even after getting vaccinated. There is also a concern related to the distribution of the vaccines since many nations are yet to receive their vaccines (Neda et al., 2016). For the nations that have received, they have failed to initiate the vaccination process as a result of the poor rollout process hence returning those vaccines to be used by other nations.

Some concerns have been raised about COVID-19 vaccines. However, this has not prevented the governments to purchase more vaccines. The members of the public are also accepting to receive the jab unlike in the previous case where many people rejected the process as a result of the potential side effects (Abazie et al., 2021). Therefore, due to the increased cases of reported morbidities and mortalities associated with COVID-19, the COVAX initiative will move forward since many people want to be secure and protected against the pandemic.

#COVAX initiative is an important approach towards fight against the threat posed by the COVID-19 pandemic. Both WHO and its partners are aiming at ensuring that there is equitable international access to #COVID-19 vaccines. It is focusing on ensuring that there is an acceleration of the development and manufacturing of COVID-19 vaccines, and to guarantee fair and equitable access for each nation. However, these objectives cannot be achieved when some countries especially those in Africa are still struggling with the rollout process. The healthcare providers in these nations must be educated about the whole process about vaccination process. It is also important to ensure that the members of the public are provided with adequate education on the importance of vaccination to help in improving the level of acceptance. This is important in guaranteeing the safety of all the individuals in the entire nation.

Week 5
Unemployment has become a severe social determinant issue facing my community today. Unemployment issues have impacted most of the Florida population as we speak because many people are out there looking for jobs. Unemployment has impacted the population to a point where individuals are suffering from financial stress. Others have become homeless while others are engaging in crimes. Most of these citizens are suffering from nutritional diseases due to poor nutrition. These effects keep on increasing the more people stay unemployed. And reports have stated that unemployed individuals seem to display issues like depression, anxiety, worry, and low self-esteem. The same individuals are also suffering from demoralization and physical pain (Health People.gov). Due to unemployment, people of unemployed Florida, are unable to pay for proper medical services, which is why such people have serious health issues. Due to unemployment, these people cannot only pay for reasonable medical services, but they also cannot afford health insurance and proper sanitation. Crimes that we mentioned above involve drug abuse resulting from the poverty caused by lack of employment. Drug abuse is detrimental to someone's health.
As a result, the type of healthcare policy that I will advocate in this situation is public health policy and health equity. This is because by addressing insufficient social and material resources, also known as social determinants of health, the correct public health policies and programs can break the vicious cycle of poverty, disadvantage, and ill health (SODH) (Williams, et al. 2018). In our capacity as professional nurses, it is our responsibility to advocate for such concerns because, at our core, we are deeply concerned about the social, physical, and emotional needs of the poor and disadvantaged people in our communities.
I intend to launch a campaign that will include local healthcare facilities and the general public, after which we will all come up with shared specific goals that will improve social and economic conditions. In turn, these goals improve the physical, emotional, and mental outcomes of the poor and unemployed in Miami, Florida. I plan to bring these common goals to the attention of policymakers at the national level to secure help for the residents of my hometown. I shall not rest until I see that there has been a significant improvement in the healthcare system and living circumstances in Florida.

It is hard to talk about unemployment without mentioning the effects it has on someone's health. Poverty is a result of unemployment, and poverty leads to improper medical services. The health issues caused by poverty resulting from unemployment are a big concern in Florida. Therefore, I feel like our duty as citizens of Florida, is to advocate for public health and health equity policy that will also cover both employed and unemployed individuals. We must make sure our neighbors are in good health, no matter their financial circumstances. The government and other healthcare stakeholders should also make it their concern to combat the issue of unemployment for the people of Florida.

Examine a nursing organization to determine the political function of the organization.
One of the nursing organizations identified is the American Nursing Association. The organization is committed to the provision of the best evidence-based practice, advocacy, policy, products, and professional development that will ensure the desired transformation in the delivery of healthcare services is achieved. The organization further seeks to advance the nursing profession and improve healthcare for all individuals. The organization plays a political function by amplifying the voices of nurses by lobbying executive agencies and congress. The organization further provides tools and offers information that empowers nurses to share their expertise and perspective directly with policymakers. The organization continues to advocate for nurses, educate and forge new relationships with congressional staff and members of Congress which enables advancing the policy agenda of the organization that seeks to support nurses and patients.
Discuss three legislative initiatives supported by the organization.
Some of the legislative initiatives supported by the organization include the nurse staffing improvement act, the Bipartisan gun Bill, and the Mainstreaming Addiction Treatment Act of 2021. According to Staebler et al., (2017) adequate nurse staffing is associated with improved patient outcomes as well as an increased level of satisfaction for both staff and patients. The American Nurses Association (ANA) has been fighting to lower dangerous nurse staffing levels for more than two decades to enhance nurses' working conditions and produce the best possible patient outcomes for their patients. The elimination of damaging nurse staffing practices and policies will help to enhance the overall quality of health care for everyone. ANA fully supports the nurse staffing improvement Act which will oversee staffing problems addressed.
ANA continues to extend support for the Bipartisan gun bill which requires individuals to undergo a background check before purchasing a firearm (Scott & Scott, 2020). Nurses are among the frontline respondents of gun violence and get to witness the trauma that violence from guns gets to inflict on the victims. ANA continues to call on policymakers and congress to enact additional meaningful reforms that will oversee the protection of the general society. ANA is also in support of the Mainstreaming Addiction Treatment Act of 2021 which seeks to further contain the opioid crisis. The above bill targets elimination of the separate requirement for registering before dispensing narcotic drugs such as buprenorphine which is crucial in the treatment of opioid dependency.
Define your position on each of the initiatives and whether you agreed with the organization's position and advocacy platform.
I am in support of all the initiatives taken by ANA in the above-mentioned legislation. The nurse staffing improvement act will ensure adequate nurse staffing which translates to improved patient outcomes. I also support the organization’s stance on the Bipartisan gun bill. I believe that conducting background checks before allowing an individual to obtain a firearm is a step in the right direction from preventing some of the acts of violence from guns. I also support the position of the organization on the mainstreaming addiction treatment act of 2021. Buprenorphine is instrumental in treating opioid dependency and its only rights that there is no unnecessary barrier placed in the ways of nurses who play a crucial role in treatment plans adopted.

Week 7
The best way to assess the impact of various injuries and diseases on demography is to conduct a burden of disease analysis. The Global Burden of Disease (GBD) represents a tool that quantifies health loss from hundreds of diseases, risk factors, and injuries to improve the health systems and aid in eliminating healthcare disparities. It adds the years of healthy life lost as a result of living with illness (non-fatal burden) to the years of life lost as a result of dying prematurely (fatal burden). The Global Burden of Disease provides a logically coherent indicator of population health that can be used to compare public health by geographic location and across time, as well as to analyze the relative burden of different illnesses and injuries. Every country of the world has the mandate to map out their population health by determining the risk factor of health that enables them to develop a measure to curb it. The United States of America is also part of this measure and implements it through its federal states. As a citizen of the United States of America living in the state of Florida, Miami, I had the chance to look into the health data of the city through the City Health Dashboard, and I easily noticed some of the health factors or diseases that continue to wreak havoc to my community. The three major diseases that affect the citizens of Miami, Florida, include; breast cancer, covid19, and cardiovascular diseases.

The state of Florida is aware of the continuous damage that breast cancer has brought upon its citizens and has developed national measures to reduce the spread and reduce the risk factor of breast cancer in the state. Florida adopts a holistic approach to cancer control initiatives to minimize inequalities, illness, and mortality associated with cancer. Such measures include; The development of Florida Cancer Plan 2020-25, which contains 21 objectives divided into eight sections that address collaborative effort, statistics, preventative measures, and risk reduction, assessment and early detection, care and management, life quality, survival rates, and end-of-life care, childhood cancers, and research and technology development. This measure provides a holistic approach to the detection and treatment of cancer.
The COVID-19 pandemic represents one of the deadliest diseases that brought the world to a standstill. What makes covid19 more dangerous is Because it is a virus that has not been seen in people, no one is resistant to it. That, along with the fact that it spreads as quickly from person to person as influenza and affects the upper respiratory tract. Some of the measures the state has implemented to curb covid19 include campaigns to sensitize the virus pathogenesis to the people, provision of sanitizers and hand wash, sensitizing the need for wearing facial masks, and maintaining a social distance. Lastly, just recently, the state has also been at the forefront of immunizing its citizens.
Lastly, cardiovascular diseases have continued to wreak havoc on the citizens of Florida.
