Spectrum Young adult program is designed for individuals who are currently battling with substance used. During this program individuals are seen by clinicians 3x a week and are encouraged to utilize the computer on their free time for any interest in employment. During visits with their clinicians, clinicians will implement CBT and other skills that will benefit the client. According to spectrum they employ CBT to help the client to participate during sessions and during groups. CBT is also used to help client with tools to sustain long term sobriety. Skills learned during this program are used on a daily basis and emphasized to ensure clients get the best service.

This program will benefit Leslie who is currently being served due to her past history using substance use. Leslie has reported drinking excessively due to peer pressure. Leslie Is underage will need a legal guardian to confirm for her to attend this program. According to the state of Massachusetts " law 262CMR8 states "Where the client or prospective client who is to receive the mental health counseling service is not mentally competent to give legally valid consent for the performance or provision of that service or function, the Licensed Mental Health Counselor shall not perform nor attempt to perform that service or function without the prior written consent of an individual who is legally authorized to give consent on behalf of that client or prospective client, or of a guardian appointed by a court of competent jurisdiction to act on behalf of that client or prospective client. Where the client or prospective client who is to receive the mental health service or function is a minor, the LMHC shall make and document reasonable attempts to obtain informed consent from both parents when custody is held jointly, or from the minor's legal guardian(s) unless the minor”. Due to leslies parents not being together it is a duty of the counselor to ensure that one parent is on board with her seeking further treatment. According to the state of Massachusetts it is important to make sure that the client has consent from legal guardian prior to proceeding with after care. In Leslies case her mother is her guardian and she currently live with her. On an ethical note considered her parents are bilingual it would be helpful for them to receive a translator. It will also be important as counselors to ensure her parents are aware of where she will be attending and the services they provide. If Leslie and her family considers this facility is a best first as counselors, it’s our duty to inform them to complete ROIs prior to proceeding to protect both the client and clinician. The ROI is information that the client will provide and consent to refrain from releasing their information to others unless it’s a facility that they will be going to for after care.

Partial Hospitalization

Agape is an intensive group home serving both boys and girls at the age of 5-21 with physical, medical, developmental limitations. Agape is also known for individuals with others trauma and other histories of abuse. Agape believes that each client that comes to the facility will be seen by a clinician as needed. Agape provides each client with clinician who are very talented and experienced and that implement treatment plans that best fit each client. Agape implement CBT to each client served as fit to better serve and formulate for client battling with depression and any other mental issues. Agape believes that using CBT as a psychotherapy to help client to be more engaged in sessions and to help client reshape inaccurate negative thinking.

Agape Inn in Holbrook, MA opens 7 days a week

The client served is in the age range of Agape. As a counselor it is our duty to protect and provide each client, we serve with the best service to those we serve. Leslie is a minor and requires ROI for consent prior to proceeding. This program will provide Leslie not only with skills to pursue long term sobriety but also social skills due to being around other peers. Agape focuses on supporting clients and their family while in their program for a great outcome. While in the program, Leslie will meet with clinician and will be advised to see medical attention if necessary. Leslie will also attend therapy where she will be practicing CBT and her clinician will be creating a treatment plan according to her need. Due to Leslie being bilingual she will probably need a translator during admission. "In code A.1.a. The primary responsibility of counselors is to respect the dignity and promote the welfare of clients". Although it is our duty to protect and promote the best services for our client, we also have to ensure that we grant access from proper party prior to proceeding and that the facility being considered is a best fit. In the state of Massachusetts code CMR8:03a states “except as otherwise provided by law, all communications, including electronic communications, between any licensee and the client(s) to whom the licensee has rendered professional services shall be deemed to be treated as confidential information in perpetuity”.

Spectrum outpatient health clinic is designed for individuals with mental health issues along with substance use problems. Spectrum believes by practicing psychotherapy and implementing CBT can lead client back to long term sobriety. Spectrum outpatient program provides counseling provides treatment plan and prior to leaving the program clients will be provided with skills needed for recovery. Spectrum also believes that cognitive -behavioral treatment will help clients the proper tool to motivate and keep clients active during their sessions with clinicians and a successful outcome.

This program is located in Weymouth Massachusetts and open 5 days a week. This outpatient clinic will be perfect for Leslie considered she is battling with substance use. According to the state of Massachusetts 262CMR8 "Where the client or prospective client who is to receive the mental health counseling service is not mentally competent to give legally valid consent for the performance or provision of that service or function, the Licensed Mental Health Counselor shall not perform nor attempt to perform that service or function without the prior written consent of an individual who is legally authorized to give consent on behalf of that client or prospective client, or of a guardian appointed by a court of competent jurisdiction to act on behalf of that client or prospective client”(mass.Gov,2021).

This program is designed for adolescent with substance use and it also issues with various stages of recovery. Prior to admission the client will complete assessment with the nurse then will receive a tour, an assigned clinician and will be seen 3x a week to work on a treatment plan. Services at this program is designed to each client to teach clients skills needed for recovery. In this program they work closely with family which will be beneficial for Leslie considered she is a minor and it’s her first time. In the event, Leslie considered attending Spectrum her parents would be task to sign a ROI for consent. Leslie would also be attending groups at least 4x a day on topics related to substance use and life skills. Leslie would also be meeting with a care coordinator that will explore different options for after care. This program provides several mental health programs that intensive including clinical assessment and medication management. Due to Leslie being a minor counselor will have a written consent from her legal guardian prior to proceeding. According to the state of Massachusetts, counselors are task to make parents aware of details where the client will be attending in depth details. In the ACA code of ethics it states B.5.a. Responsibility to Clients (ACA,2014) "When counseling minor clients or adult clients who lack the capacity to give voluntary, informed consent, counselors protect the confidentiality of information received—in any medium—in the counseling relationship as specified by federal and state laws, written policies, and applicable ethical standards. Due to Leslie being a minor it is our duty as counselors to ensure either guardian if both parents are not present to ensure they comprehend and are clear of the process.

Mclean hospital is an inpatient program located in Boston Massachusetts and is open 7 days a week. At McLean Hospital they believe in the theory of psychotherapy to which is family focused and interpersonal theory to help clients map out a road mad that specifically work for them. During session clients will meet with their clinician and create a treatment plan that is designed for them and during this session clinicians will also with client to emphasize both theory family focused and interpersonal. Mclean hospital believes using the theory family focused considered these clients are minor will help guide them in the right path and also help the clinicians to have in a place a safe boundary for them if necessary. While minors aren’t capable to make their own decisions as minors it’s important that interpersonal boundary is placed to focus on reducing stress. Leslie is a minor who has a history of consuming alcohol. Leslie may also suffer from some other underlining issues considered she does not have any contact with her biological father and is not close to her stepfather. According to this facility they specialized in trauma which would be great for her. Leslie is a minor and in order to proceed we as counselors will have to consent the legal guardian and they will be task to sign a ROI. If Leslie was to consider this program, she will benefit from group therapy, medical attention and 1:1 session with a trained clinician where they would create a treatment plan designed for her to address any issues. At Mclean they are dedicated to improve the live of each individual by providing the highest quality of service effective clinical care and more. According to State of Massachusetts regulations law code 262CMR803:1 “Licensed Mental Health Counselors must inform clients, in writing, of policies regarding confidentiality of information and the legal limits and exceptions to confidentiality. Licensed Mental Health Counselors shall not communicate either verbally or in writing with others about a client without the client's express written consent, including any legal proceedings, except when the limits of confidentially may legally be invoked, such as, but not limited to, cases of potential harm to the client or significant or deadly harm to others by the client, and legal proceedings under M.G.L. c. 112, § 172”.according to the ACA code of ethics code A.2.d. Inability to Give Consent “When counseling minors, incapacitated adults, or other persons unable to give voluntary consent, counselors seek the assent of clients to services and include them in decision making as appropriate”(ACA,2014).

c. Partial Hospitalization

Pembroke hospital is a partial hospitalization located in Pembroke Massachusetts. At Pembroke Hospital they believe in family focused therapy. Pembroke hospital is very big on involving family in clients service as long as they have an ROI in place. Pembroke is designed for clients in the age range of 13-17 years old struggling with various mental issues. At Pembroke hospital they have designed a schedule for family to visit for support for their loved ones. This program finds it important to practice family focused because clients who are underage will go through different emotions like sad and many more. It’s imperative that each client have the support of loved ones while getting treatment and in the journey of becoming their best self.

Leslie was able to identify her triggers during session and as a counselor it is my duty per the ACA code of ethics to provide resources for each client served. Code A.2.b. Types of Information Needed” Counselors explicitly explain to clients the nature of all services provided. They inform clients about issues such as, but not limited to, the following: the purposes, goals, techniques, procedures, limitations, potential risks, and benefits of services. Leslie and the counselor along with her legal guardian will be informed of the services provided to prior to signing the ROI. Although she is a minor, she still has a right to comprehend the details of the facility. At Pembroke hospital each client will leave with the skills to deal with crisis. At Pembroke hospital each client will learn the skill to evaluate their needs and with clinicians help the client while in treatment by providing motivating them to tap into their potential self. each client and family will receive a handbook with details of services and mission statement. At Pembroke hospital psychiatric services are provided along with group therapy, coping skill, medication management and other services as it fists in the client treatment plan. According to the state of Massachusetts code CMR8:02c12345 “ Where the client or prospective client who is to receive the mental health service or function is a minor, the LMHC shall make and document reasonable attempts to obtain informed consent from both parents when custody is held jointly, or from the minor's legal guardian(s) unless the minor: 1. is emancipated by court petition and decree; 2. is married, widowed or divorced; 3 is a parent of a child himself ”. Due to Leslie being underage her guarding will sign the consent forms if that is the route they chose to take. As counselor I will complete all necessary paperwork prior to her leaving including resources for aftercare.

d. Outpatient

DCS mental health is located in Raynham, Mass and it is designed for child adolescent psychiatry and other mental health issues. DCS is known for counseling services, for children, adolescents, social behavioral issues. They are committed to using cognitive therapy and interpersonal to help best serve each client. At DCS CBT is used during session and also when clients meet with their psychiatrist. Each client that is at DCS will leave with skills to write goals and the importance of achieving them on their own terms. DCS also implements skills like journaling especially when clients are having a difficulty from being away from home for a long period of time. DCS also meet with trained clinicians that provides clients with positive activities after their session to help each client overcome obstacles and improve on coping skills. During groups interpersonal therapy is used to help client gained proper skills they can have after completing the program. After speaking to Leslie about DCS it sounds like she is interested in the program. Due to her being a minor her legal guardian also has to be on board. Leslie will complete with her guardian an ROI for consent prior to proceeding. Leslie also undergo some is underlining issues with her biological father not being present. During sessions at DCS Leslie will be able to attack these issues with the help of their psychotherapy.

At DCS they provide group therapy, and 1:1 session for each client. They also provided skills like mindfulness, interpersonal skills, effectiveness, regulations and more. According to ACA code of ethics code A.2.c. Developmental and Cultural Sensitivity “Counselors communicate information in ways that are both developmentally and culturally appropriate. Counselors use clear and understandable language when discussing issues related to informed consent” (ACA, 2014). Although Leslies mother speak English well, it is important that as a counselor it is our duty to speak clearly and in plain English refraining from certain words in order for parents and the client to better understand.
