PICOT Question: For adults with high cholesterol (P) does a daily 30-minute exercise regimen (I) reduce the levels of cholesterol (O) compared with no exercise regimen (C)?

Qualitative Studies

Higgins, J. & Higgins, C. (2016). Prescribing exercise to help your patients lose weight. Cleveland Clinic Journal of Medicine. Volume 83 (2): 141-149.

Wang, Y., Xu, D. (2017). Effects of aerobic exercise on lipids and lipoproteins. Lipids in Health and Disease. Vol. 16: 132.

Background of Study

In Wang and Xu (2017), the researchers embarked on solving the problem of finding the most effective way of controlling cholesterol through regular exercises that have an impact on lipids and lipoproteins in the place of the traditional approach of using medicine. This is an important article in the nursing profession because it provides another approach that aids in improving patient health. Furthermore, it helps in empowering the process of improving their health. The purpose of the study was to look for other long-lasting and reliable option that does not require a patient visit to health facilities. The research question on the study was how the incorporation of exercises in the treatment of dyslipidemia can help in reducing myocardial infarctions.

The second article by Higgins and Higgins (2016) disuses how simple regular aerobic exercises lasting between 30 and 45 minutes helps in the process of improving insulin sensitivity and lipid level. It addresses issues that are highly important for the nursing profession. Among the most important issues discussed is the incorporation of aerobic exercises not just for controlling cholesterol but for the overall well-being. The purpose of this study was to evaluate the effectiveness of regular aerobic exercises in overweight people for effective management of health problems they face daily. The research question was how comprehensive exercise programs can help in long term weight management and health problems that arise from this risk.

Support of PICOT How do these two articles support the nurse practice issue you chose?

Vol. 16: 132.

The nursing issue chosen supports how daily 30-minute exercise regimens reduce the levels of cholesterol among adults with high cholesterol. The two articles will effectively help address this issue. Higgins and Higgins provide various forms of medication and treatments that can be undertaken in the process of lowering cholesterol levels in adults. The impact of these procedures is then compared with results with the process of exercising, which comes out as having the greatest long-term benefits for the subjects. The article, therefore, helps in the question chosen by providing several forms of 30-45-minute exercises with a positive impact in reducing cholesterol. Aerobic exercises are specially mentioned as having the biggest positive impact among subjects in the study. Comment by ESC: APA citations Must provide citations so; 1) it is clear to the reader which source you are talking about, 2) to ensure proper credit is given, and 3) to avoid plagiarism APA in text citations - https://owl.purdue.edu/owl/research_and_citation/apa_style/apa_formatting_and_style_guide/in_text_citations_the_basics.html APA in text et al https://owl.purdue.edu/owl/research_and_citation/apa_style/apa_formatting_and_style_guide/in_text_citations_author_authors.html

The article by Wang and Xu is also especially important in addressing the chosen PICOT question since its findings support the importance of incorporating exercise in the treatment of cholesterol. This is in the understanding that exercises have the most positive long impact on the patients. Furthermore, it helps in ensuring that lifestyle change is undertaken in the process of living more active lives. Wang and Xu's article also provides great advice on how health professionals should approach and advise their patients on the importance of exercising as part of the medication process for cholesterol control. Both articles, therefore, contain important information to help in dealing with the issue and provide long term solution that involves patients taking the matter into their own hands without much reliance on medication.

Method of Study Comment by ESC: Incomplete Methods A thorough discussion on the method of study for each article is presented. The comparison of study methods is described in detail. A benefit and a limitation of each method are presented. The discussion demonstrates a solid understanding of research methods. Identify the type of qualitative design of each study: 1) phenomenology 2) ethnography 3) grounded theory 4) case study Neither article you provided was qualitative design

In Wang and Xu's article, the methodology used involved an analysis of past studies that had been taken previously on the impact of exercises on cholesterol levels in humans. The authors cited four different studies. One study consisted of 12 women who were involved in 16 weeks of exercise. The second study consisted of 36 men who underwent 12 weeks of exercise each week consisting of 150 minutes.

The third study involved 111 men and women on 24 weeks of exercise that lasted at least 24 minutes daily. The last study consisted of 64 men on 24 weeks of observation. In Higgins and Higgins, the journal article embarked on the process of evaluating 35 different studies on the issue of how exercising daily for 30 minutes impacts on reducing cholesterol levels. In all the studies, it was found that daily exercising resulted in reducing cholesterol levels even when there was no change in diet. The impact was higher on subjects whose exercise regimen was accompanied by a change in diet by choosing healthier meal plans and sticking to them.

Both articles, therefore, have a similar approach since they embarked on evaluating other studies completed previously. The choice of secondary research is highly effective because researchers can compare the impact of exercises over a larger sample size, which would have been cumbersome had they chosen to use primary research approach. Furthermore, the wide subject base makes it possible to compare the impact on different demographics than in other competing types of research.

Results of Study Comment by ESC: Incomplete Results Discussion of study results, including findings and implications for nursing practice, is thorough with substantial relevant details and extensive explanation. Qualitative results are noted as common themes, which result from transcription of data. If a study results notes statistical data, then it is quantitative design . Qualitative Results – Themes For qualitative research, the findings are reporting by presenting the common themes of the analyzed transcribed data. Make sure to provide this info in the Week 5 paper, see attachment for additional feedback about themes http://designresearchtechniques.com/casestudies/thematic-analysis/

In Wang and Xu, the findings were that aggressive lifestyle changes that include daily aerobic exercises help in reducing cholesterol as well as other life-threatening complications developing from high cholesterol such as myocardial infarctions and stroke. The study further finds that impacting the knowledge on the affected patients through the empowerment approach helps deliver positive results because of encouragement. In Higgins and Higgins, the findings hold that best results are gained through long term sustained exercises. This means taking complete lifestyle change positively and embarking on taking simple exercises to avoid burnouts.

The results of daily aerobic exercises go beyond lowering cholesterol and into wider benefits including better functional status, weight loss, and suppression of risk factors for cardiovascular ailments. Both studies are highly important in the nursing profession because they provide affirmation for what has long been seen as an important element of controlling cholesterol in humans. They further confirm nursing professionals’ role in ensuring that individual patients live better lives by empowering them with knowledge on how to achieve that important goal.

Ethical Considerations Comment by ESC: Incomplete Ethics Discussion of ethical considerations associated with the conduct of nursing research is thorough with substantial relevant details and extensive explanation. A detailed discussion on ethical considerations of the two articles used in the essay is presented. Use Correct Terminology Ethical Principles Principles of research ethics Minimizing the risk of harm. Obtaining informed consent. Protecting anonymity and confidentiality. Avoiding deceptive practices. Providing the right to withdraw Should also mention IRB approval and the organization that granted the approval

Ethics in research is essential in ensuring that researchers do not end up hurting the very people who are also the intended beneficiaries. For instance, naming subjects by their names in the study would lead to having their identity being known by readers. Respect for privacy is crucial in all kinds of studies. Secondly, body shaming can result in hurting the overweight individuals who are subjects in the two studies. This can also hurt them to the point of refusal to participate in future research and even loose morale to improve into healthier and active lifestyles. Both studies have respected these ethics very well.
