Diabetes mellitus is considered the leading cause of chronic kidney disease (CKD). It is estimated that 40 percent of the patient diagnosed with diabetes developed diabetic kidney disease (DKD) hence leading to albuminuria, reduction in the glomerular filtration, or both. Therefore, the assessment process for the organization will involve looking into measures that have been put in place to ensure that the risk of the patient diagnosed with diabetes developing CKD is reduced (Chen et al., 2019). Some of the important measures include the early screening of both children and adults for diabetes development and those who are highly exposed to the risks of being diagnosed with this condition for example individuals with a family history of diabetes disease.

The assessment process also looks into some of the health education measures or programs that are aimed at informing the community members about some of the risk factors associated with kidney disease development, the importance of a healthy diet, and the physical exercise involvement to reduce the risk of exposure to CKD. The assessment process also looks into measures that are put in place by the healthcare providers in the facility for monitoring the medication adherence, the follow-up process, and whether there is an implementation of the home-based care intervention using telemedicine. Some of the available opportunities and the stakeholders to be included in addressing the issue of CKD as a result of diabetes must also be looked into. Some of the available resources can include the community leaders, the nurses, and teachers in the schools to ensure that both parents and children are informed about the risk of developing CKD as a result of the poor management of diabetes and other important measures (Gregg & Hedayati, 2019).

The ultimate goal for the DNP project is to ensure that patients who are already diagnosed with diabetes acquire knowledge and skills on the successful management of their diabetic conditions. It is also aimed at promoting medication adherence among patients to ensure successful management of their conditions. It is focused on increasing awareness in the facilities for nurses to increase the level of patient education at the community level and facility level to reduce the risk of complicated diabetes conditions such as CKD (Buse et al., 2020). These goals meet the vision of the organization that is focused on reducing the morbidity and mortality cases of preventable chronic illnesses.
