A session and a case note
Overview
The session case note assignment is designed to allow you to practice writing a case note for an actual live therapy session. This hands on assignment allows you to identify what elements of a session should be noted in a case note. This assignment will be completed through a case note template and will be worth 50 points.
Assignment Instructions
For this assignment you will watch the entirety of the therapy session in the link below. The session begins at 25:30 and ends at 1:10:00. After you have watched the entirety of this family session, you will complete the case note template for one individual in the session. In the case note template, each section must be completed as if you were the counselor/therapist of this family session. It is important to note if you are in the counseling certificate program, you will complete the case note by use of your theoretical orientation. Keeping in mind ethical considerations as you decide what to include in your case note. The case note should have all of the following components:
Date
Client # (you can make this up)
Session #
Clients Present
Length/Modality of session
Identified Ethical Concerns and citation of ethical codes to justify the concern
Goals of the session
Intervention Domains and Notes:
In the notes/narrative portion of the case note it is important to note models/theories used, interventions, important content within the session, and client’s demeanor.
This case note template should be completed in it’s entirety as if you were the counselor/therapist who conducted this session. The assignment is due by 11:59pm CST on Day 7 of the week.
