Initial call transcript from the patient’s point of view
I am a husband of one child and have stayed in the marriage for the last 34 years. I have been working for a New Jersey Company for the last 32 years. My life completely changed when my brain stopped working properly. At the start, the problems seemed small, hardly noticeable. Life was good before, until my brain went downhill, taking the whole family down with it. I have been a dedicated father who loves his family. I realized things have started to change when I forgot about my anniversary. My wife was furious that that day and I hardly noticed her anger and reaction until she reminded me about the anniversary. Forgetting even very little things made me visit the psychiatrist. Also, I could the plates on the floor after eating, spilling the food all over me, and unable to drive when am heading to work. I needed to visit the psychotherapy to know why the brain has been not functioning properly.
What would the patient say?
The patient would not maintain a smooth conversation
The patients keep on saying he is not sick
The patient’s mood would change when he continues to speak
What tone of voice might he or she use?
Using a positive and friendly tone
A low/calm tone (pitch)

The patient would speak in a slow-paced tone
The patient misspells words, for example instead of decision he says revision

The message is not understandable
It is circumstantial because the speech has non-linear thought pattern

The patients made me feel like am experiencing dementia myself
I feel overwhelmed and irritated by frequent repeat of questions.

Outline for the Psychiatric Diagnosis
1. Introduction
A. Thesis: mental treatment considers the potential justification on the use of ideal concepts of therapy with focus on the a patient’s life situation, environment, and all incident factors, which affects the normal mental functioning by using cognitive therapy and ensuring adequate contributions to patient clinical assessment and diagnostic.
2. Body Paragraph 1: mental diagnosis is a creative analysis of any changes in the patient’s life. These changes helps to understand the different activities, issues, and ways to ensure productive mental health and performance.
A. Supporting Evidence: according to Shou, et.al. 2017, the application of different treatment protocols and methods helps to understand the incident factors. The focus on the incident factors is choosing the right ways, methods, and approaches towards effective mental health diagnosis.
i. Explanation: there is value on understanding the effects of the patient. These effects paves the way for an effective medical diagnosis and mental assessment.
ii. So what: the explanation supports the creative application, understanding and importance of creating a creative processes for managing to influence the changes and impacts of ineffective mental assessment.
3. Body Paragraph 2: through effective counselling it is important for a practitioner to develop a productive environment, which will help in rolling in the patient to a comfortable interactions.
A. Supporting Evidence: according to Feinstein, Dyar & Pachankis (2017), it is imperative to develop an engaging environment to use, develop, and provide an appropriate environment.
i. Explanation: the value of a productive environment is engaging and productive interaction between the practitioner and patient.
ii. So what: it is important to use, develop, and influence a patient, especially, when affected by mental health. An engaging environment helps to improve the relationship and mental health.
4. Body Paragraph 3: Choosing one of the many cognitive based therapies is ideal and will help in developing a treatment plan and strategy for the patient.
B. Supporting Evidence: based on O’kane (2010), there are different techniques, which will help in focusing on the challenge affecting anti-social changes and behavior. The application of these techniques are core and critical for effective therapy processes.
i. Explanation: social behavior changes influences some of the conditions, which affects cognitive behavior and communication.
ii. So what: it is also important for an essential communication to help in dealing with the social changes in the community. The focus on how to ensure treatment can be achieved through effective therapy treatment.
5. Justification of the chosen manual: the treatment improvement tool (TIP)
C. Justification: the use of the tool is focusing on the importance of the treatment process. It helps to provide the foundation of focusing on effective recognition of the value of the treatment process. It can be used during the process of therapy or during and after the treatment process.
6. Evaluation of the Symptoms: Understanding the incident factors of the mental condition
D. Justification: closer to understanding the close change in the normal activities and conditions, also, influences how the mental health affects an individual. The use of work distractions and lack of concentration is also an imperative lack of motivation with little considerations on emerging issues. More importantly, it is also a solution for overcoming the common changes, ways, and issues related to effective treatment and recovery.
