Triple therapy was formed to improve the health of the individuals. It consists of improving the population health, per capita cost, and care experiences(Endo, 2016). Most nations are faced with the three problems where health is expensive because most of the population has a chronic illness that is expensive to diagnose and treat. Indeed expensive health care results in poor quality of care. In the US, for example, the majority of the patients are older adults with chronic illnesses which are expensive to manage and treat. One of the chronic conditions includes diabetes, which affects people worldwide, the majority being the old generation. According to CDC tracking data, with the onset of the Covid 19, the management of diabetic patients has become expensive. Indeed, it is relevant to appreciate the measures of the Triple Aim.
Therefore, there are several ways to consider when measuring the Triple Aim. The elements that are needed include the population, including the people living in certain area. The data tracking overtime is also needed for the identification of relationships before interventions and after interventions. The improvement of the population's health can be measured by monitoring the population over time(Endo, 2016). It will help identify the burden of the disease and the outcomes of care. The per capita is measured by assessing the total cost of health care every month. The last triple Aim is the care experienced and is calculated using surveys to determine the quality of the healthcare.
In my community, most of the sick people are older people with diabetes. Therefore, to measure the triple Aim, my community identifies the high-risk population which isthe aged persons. Their needs are identified and how they influenced them. Over time they are tracked and assessed to determine the deterioration and improvement of health (Obucina, et al., 2018). Also through tracking the total expenditure on the health needs gets passed. Importantly, the total cost gets assessed based on the quality of the health care provided. By so doing, the community achieved the Triple Aim. Improving the quality of health, reducing the cost of expenditure, and improving the outcome of health increases the quality of life in the community.

The components of Triple Aim include advancing population health, enhancing the patient experience of care (this also involves quality and satisfaction), and decreasing the per capita cost of health care. There are various ways through which Triple Aim can be evaluated. Population health, for example, can be assessed by looking at health outcomes, including mortality rates, health and functional status, and healthy life expectancy. Population health can also be assessed through the occurrence or prevalence of chronic conditions and behavioral and psychological aspects such as smoking, alcohol use, blood glucose, and blood pressure (Endo, 2016). Care experience can be assessed based on effectiveness, quality, equitability, and patient safety. Notably, patients play an essential role in measuring the experience of care. Per capita cost is assessed through significant lenses, namely "providers' supply lens," "the purchasers', public and clients’ lens," in addition to the intermediate lens of health plans and guarantors.
One major health problem in Florida is heart disease. According to the Centers for Disease Control & Prevention (2019), heart disease is the primary cause of death in Florida. In Florida, the population’s health is measured using the mortality rate, health and functional status, illness load, and psychosocial and behavioral aspects. The 2019 statistics provided by the CDC indicate a death rate of 200 and the number of deaths at 46,279 due to heart disease. The psychosocial and behavioral factors of the population include smoking, physical inactivity, and the use of alcohol.
One area that needs improvement to accomplish the Triple Aim is advancing the population’s health. To improve the health of the population, it is crucial to advance prevention of illness and health promotion, as well as restructure primary care services and systems and engage people and their families when designing care models. Healthcare organizations should increase their focus plan care to suit the population’s needs.
