Part 1: My Career in Three to Five Years

My desired future role is to be the Health Services Director in a skilled nursing facility

(SNF). I hope to achieve this role because it is a role that will allow me to manage the day-to-day

operations of the facility and still allow me to help patients in a bigger sense, rather than working

with them hands-on. As a health services director, I would like to make a change and help

improve the quality of care and service, as well as ensure that the facility operates smoothly. I

chose this setting because I have previous experience in an SNF as a certified nursing assistant

(CNA). I enjoy the skilled nursing environment because it is filled with a variety of patients,

whether they are there for short-term or even long-term care.

In order to achieve the role of a health services director in an SNF, I will need to attain

my bachelor’s degree in health services management and may even continue on to attain my

master’s degree in healthcare administration as well. I will achieve my bachelor’s degree by next

year, and it will take me 2 years or less to attain my master’s degree. I will need to pass a

background check, have 1,500 training hours (for the state of Washington), and pass the long-

term care national licensure exam in order to be licensed as a nursing home administrator. After

doing some calculations, I found that it would take me roughly 3-4 months to attain 1,500

training hours if I trained 40 hours a week. Most skilled nursing facilities require 5-10 years of

experience working the healthcare field, and some require supervisory experience. I do not

currently have any supervisory experience; however, I do have about 3 years of experience as a

CNA. I will have all the requirements to become a health services director for a skilled nursing

facility by 2026.

March 7, 2021 I have identified my professional style based on management and leadership theories.

Deming’s 14 Points of Management can be applied to my desired future role by helping me

improve all aspects of the facility’s work (Neave, 1987). Because all 14 points are concepts of

teamwork – between managers and workers, different departments, the organization and their

suppliers, it will help me become a better health services director (Neave, 1987). As for

leadership, I plan to embrace the leadership style based on Theory Y. Theory Y is a

consultative/participative style of leadership that includes loose general supervision, trust, shared

decision making, extensive involvement of workers, open communication, and internal rewards

(Olden, 2019).

As a health services director of an SNF, I hope to improve the quality of care and services

provided within the facility. I hope to make a change in the patients’ lives and be able to provide

the best quality of care possible. I would also hope to keep all my employees happy and satisfied

by creating a healthy and positive work environment. I hope to create a work environment that

provides amazing opportunities and has the possibility for growth for employees. With all that I

hope to accomplish, I also hope to establish a great reputation for the facility.

The Function of Planning

The SMART goals template from Smartsheet and the PDF on Strategic Planning from

Practice Management are both managerial tools that would be the most beneficial for my future

career. The SMART goals template is something I would look back on and reference and utilize

when planning and creating goals for the SNF. For instance, I may come up with a simple goal

for the facility such as, preventing and reducing spread of infection; however, the SMART goals

template will help me turn that simple objective into a clearer goal while also setting me up for

success.

Although the PDF on Strategic Planning is directed aimed towards a physician practice, I

found the article to be very interesting and thought that it would still be beneficial for me to

reference when planning for a SNF. The article mentions eight reasons for strategic planning,

which include vision, priorities, alignment, identify challenges, direction, open communication,

empowerment, and values and culture (Hernandez, 2020). I thought that the eight reasons that

the article discusses are great reminders to any healthcare administrator of how and why strategic

planning could be beneficial.

The Function of Organizing

The article on organizational structure and charts that discusses seven types of

organizational chart templates would be beneficial to me as a health services director. The article

includes the following seven types of organizational charts: line, staff or functional authority, line

and functional, matrix, divisional, project, and committee (SlideUpLift, 2019). I figured that this

would be a helpful resource to be able to reference as health services director. It would be helpful

when establishing the chain of command within the SNF. For instance, I may find that in a

March 7, 2021 skilled nursing facility, a functional organizational structure is the best option and could

reference this resource when constructing an organizational chart. Within a functional structure, I

could organize the facility by different departments such as, nursing, dietary service,

housekeeping, maintenance, admissions, etc.

The Function of Staffing

The 30-Day Performance Improvement Plan (PIP) template from Smartsheet and the

Performance Management Guide from Society for Human Resource Management (SHRM) are

two managerial tools that would be very helpful for the staffing function. The PIP template is a

great resource to have as a manager of any type, let alone a health services director, just to have

in case there is a situation in which an employee is in need of performance improvement. As a

health services director of a SNF, I would reach for this template if I found that I had employees

that were not performing well. I would even use this template for employees that are doing well,

to further encourage employee development.

The Performance Management Guide from SHRM is a great resource to have as a

manager. It discusses performance management, why it is important, and it even mentions how

to design and implement an effective performance management approach (Dorsey & Mueller-

Hanson, 2017). This resource mentions the ideal elements of a performance management system

such as, having a simplified system that focuses on improving performance, having flexible goal

setting, emphasizing performance measurement, coaching and feedback, and having different

criteria for different decisions (Dorsey & Mueller-Hanson, 2017). I would reference this guide

often, but most especially when establishing a performance management approach that works for

myself, the facility, and my employees.

The most beneficial resource would be the American Society for Quality (ASQ) Quality

Resources website. The website has a variety of resources regarding quality and various quality

topics. This is a great resource to have as it really has everything related to quality, quality

assurance (QA), quality control (QC), etc. I would use this management tool often, as it is full of

so many other resources as well. It’s a great tool to have when establishing a quality management

system, or even when trying to perform quality control.

The Functions of Decision Making, Problem Solving, and Change Management

The Ultimate Problem-Solving Process Guide would be incredibly beneficial for the

function of problem solving. This guide includes nine profitable problem-solving methods,

fourteen great problem-solving resources, and even discusses a thorough 8-step problem-solving

process (Smith, 2017). This guide has so many resources for a manager to utilize when it comes

to problem-solving. As a health services director, I would definitely turn to this guide if I’m

stumped by a problem and I would even turn to this guide every so often to refresh my mind on

the many approaches to problem-solving. For instance, if I come across a problem in the skilled

nursing facility, I could reference this guide to identify which problem-solving technique would

work best for that particular problem.

Resources and Websites

The Centers for Disease Control and Prevention (CDC) and the Centers for Medicare

and Medicaid Services (CMS) Regulations and Guidance websites are both resources that I plan

to use in my career. I would keep up with the CDC’s website to ensure that the facility and

myself are up to date with the CDC’s guidelines. The CDC website is also a great source for any

information regarding outbreaks, emergency preparedness, workplace safety and health, etc. The

ensure that the facility is following all the necessary guidelines and procedures to ensure the

health and safety of the patients, as well as all employees.

The Regulations and Guidance page from CMS is an important resource to keep up with

as it includes many regulations vital to healthcare. It’s important for healthcare management to

keep up with these regulations as the legislations and regulations in healthcare are constantly

changing. On this website, you are able to view updates on guidance and regulations by provider,

in which I selected SNF and found “spotlights” which included updates related to CMS that are

important for SNF. This website would support me as a manager because it will allow me to

ensure that the facility is complying with all the regulations mentioned by CMS.
