In everyday practices, there are routine matters which arise in the initial contact. A routine matter is identified as a standard procedure that is usually carried out as a way of performing things. A routine matter can lead to huge challenges if not looked into carefully. In order to carry out discussion on routine matters appropriately, it is important for a physician to utilize analytical skills in enhancing diagnostic reasoning along with utilizing interpersonal skills to establish and facilitate adequate communication. Routine matters discussed in the initial contact entail the suffering as well as problems affecting the patient such as discomforts and various uncertainties of a disease (Ivey & Zalaquett, 2010).  There is also a discussion on the spontaneous report of the patient’s history whereby the patient has a dialogue with the physician on medical history as well as age and marital status. There is also a discussion on the current and previous health challenges of the patient to asses current health related risk elements. Nonetheless, there are completion of forms along with collection of insurance data, outlining of the purpose of the identified agency and attaining informational releases.

Define case review and list its benefits.

A case review is defined as the periodic examination regarding a client’s case. It can take place in different settings between the identified helper and the associated client. A case review can take place at any phase regarding the helping process however it is usually common during an assessment of the case. There are various benefits regarding a case review. A case review is identified as an integral part involved in the accountability structure involving an organization. Accountability entails being responsible for any outcome associated with a client. It is essential in ensuring effective and adequate service delivery to the identified client as well as maintaining standards concerning quality care. Case review are also essential in carrying out successful medical examinations on the client (Ivey & Zalaquett, 2010). Therefore, case reviews should be practiced to enhance efficiency and adequacy in service provision to clients.

Trace the client’s participation in the three phases of the helping process.

Client participation is referred to as the act of the client taking an active part in the identified helping process thus enhancing the responsiveness of service delivery as well as enhancing effectiveness of the helping process. Encouragement of client participation possesses identifiable components (Ivey & Zalaquett, 2010). Client participation plays a vital role in the process of collecting information as well as assessment activities. In most times, the client is considered as the primary and basic source of information through giving historical information as well as desired results and so forth. Roy is considered to be self-referred in regard to the agency in the case study. Roy was prepared and collaborative throughout the helping process. In the assessment stage he offered information concerning routine matters. In the planning phase, Roy’s plan indicated that he had eligibility for services and he had met the agency criteria. In the implementation phase, Roy worked closely with other related professionals. Roy was a good client who was flexible enough throughout the helping process which helped in the modification of his plan.

Summarize the limitations of codes of ethics.

Code of ethics have been implemented by professions in order to operationalize the various commitments to enhance provision of guidelines necessary for practice. The code of ethics therefore acts as a guideline to ethical practice. There are various limitations regarding the code of ethics. First and foremost, a code of ethics associated with a profession is considered to be binding solely to members involved in the group which adopts it. Those individuals who are not group members are usually not bound by the identified code of ethics (Corey & Callana, 2007). Nonetheless, another challenge associated with the code of ethics is that it is considered improbable for a code of ethics to offer coverage to ever probable situation which can arise during the helping process. The code of ethics highlights on confidentiality of the patient and this limits the work of the helper in cases whereby the client wants everything to be confidential. The helper gets a challenge when they have to make a report on something.

Describe this approach as it relates to each phase of the helping process.

A strength-based approach is essential in the helping processes. It bases focus on the talents as well as skills, dreams and various interest regarding a person as a form of empowering along with motivating and engaging both internal and environmental associated supports. A strength-based approach in relation to assessment phase bases its focus on the positive features along with capabilities and various encounters regarding the client in addressing current issues. In the second phase which is planning, a strength-based approach is important in enhancing effective planning. The various strengths as well as capabilities and resources regrading the client transform into being part of the plan advancement. In the third phase involved in the helping process, the implementation phase, a strength-based approach enhances forming of choices as the identified client possesses options in terms regarding the various goals determined in the planning phase (Ivey & Zalaquett, 2010). 

List the two questions that guide assessment of the collected information.

There is a dual focus regarding the assessment phase (Ivey & Zalaquett, 2010). One of the focus is collection of information regarding the needs as well as resources of the client. The second is identified as assessment of the client functioning. There are various questions which can be utilized in acting as a guideline to the assessment of the gathered information from the client. The questions act as an encouragement to recalling previous behaviors as well as situations. Some of the questions that can be utilized are:

1. Discuss with me on a time when you underwent a similar challenge?

2. What do you think is your most essential capability?

Give three examples of issues that applicants may raise during the initial interview.

There are a variety of issues which applicants can raise during the phase entailing the initial interview. Firstly, applicants can want everything about them to be considered as confidential. They can regard their personal as well as professional details which cause a challenge on both the applicant and the helper (Ivey & Zalaquett, 2010). It possesses a great challenge as it makes it difficult on making a report on a critical issue. Moreover, there is a situation whereby applicants do not want the help of the helper which also causes a great challenge. It is frustrating to watch a client experience failure especially in cases where there is a chance to enhance improvement and stability. Another example, is when the family of the applicant is against the helpers. There are situations whereby the family of the applicant tell him or her that she should not undertake medication.

What role does documentation play in the review of the problem?

A case review is defined as the periodic examination regarding a client’s case. There are various parts associated with a case review. An essential part regarding a case review is identified as documentation concerning the case. Documentation is referred as the written record about the work undertaken with the client. It entails the initial intake along with assessment of data, planning records, implementation aspects, evaluation and lastly termination of the identified case. It also entails a variety of other written reports on various aspects of the client. The particular type of documentation utilized is dependent on the nature concerning the agency as well as the provided services, the length regarding the providers and the associated providers. Documentation is essential for future reference. Moreover, it enhances organization of a client’s relevant information thus enhancing easy retrieval. It also enhances efficiency and appropriateness of file management regarding the client’s information. Documentation is essential in storage of relevant information regarding the client (Ivey & Zalaquett, 2010).
