A TIC WAS USED TO CHECK THE BOOKSHELF FOR REMAINING HEAT, AND FOR EXTENSTION. NO EXTENSION WAS FOUND. FIRE OFFICIAL GREVES PERFORMED CAUSE AND ORIGIN INVESTIGATION WITH ASSISTANCE OF CHIEF PIVNICHNY. The fire department was not dispatched to the scene; students and faculty were not evacuated nor were the alarms activated. Juvenile was identified as a suspect after Jersey City Police Officer collected witness statement within classroom 204A. Jersey City Police Notified the Jersey City Arson Unit who collected and submitted into evidence a plastic cup and gel. Jersey City Police transported Juvenile to the Juvenile Bureau at 139 Cator Ave. Juvenile was check for warrants and prior offenses which proved to be negative. The juvenile was interview in the presence of his mother. In this interview, suspect was questioned about his actions. Juvenile explained that he was curious to know what would happen with hand sanitizer if he lit it with a lighter. He asked the teacher then he lit the gel and when he saw a flame he blew it out. One witness said "I thought I saw lighting through one of the windows and when I looked back it was a fire". while sitting in chairs near the fire room in the main band hall there are three side rooms for various equipment. These side rooms have electric baseboard heaters, within the room that had the fire a base drum was hard against one electric heater and it was on (even at our arrival with the fire out). Folks within the room ran to get a fire extinguisher at which time the fire alarm went off due to the smoke, and while exiting the building a pull station was pulled, all of which was seen within the alarm panel without any issue. Once we arrived all the equipment within that room was removed including the drum which was brought outside. The drum was doused with water prior to it going outside. Upon further investigation, we found the other side rooms had one electric heater was also on (we turned it off). All that was needed is a venting of the band room and the alarm was reset without issue. Fire Inspector Follow up notes - Conducted follow up investigation. 01 violation issued for materials/equipment/contents stored within 36 inches of heater. 30 days to correct issue. Fire alarm company to inspect all heads in area that had smoke. The drum that caught fire was 60 years old. The teacher extinguished the fire using a portable dry chemical fire extinguisher. There were no reported injuries. The teacher stated this is a normal experiment for her class and this was occurs every year and has been for the last 12 years. Chief Stearn then notified myself of the situation and we both responded to the high school. The fire official Bob Westdyle was also notified Once we arrived, we were told that the arts teacher had put a clay sculpture in this small kiln, a toaster oven size and started to smoke and caught fire. The School Resource Officer noticed the smoke and immediately unplugged the device and got it outside of the school. They did report there was a significant smoke condition in the room. The teacher involved was not present when the chiefs or fire official were at the school investigating. It was reported that the teacher had inhaled smoke and was transported by a family member to the hospital. The kiln was plugged in and on a shelf surrounded by other combustables. We questioned the SRO, Vice Principal, and principal why the fire alarm pull station was not pulled and why we were notified roughly 2 hours after the incident. SCENE INFORMATION: I met with Deputy Chief P. Drennan B group who stated that the incident was first dispatched as an alarm activation and was upgraded to a working fire in room 310. Sprinklers were activated and a 1 ¾ line was stretched for extinguishment by Engine 19who found the door to the room unlocked. Upon my arrival there was no visible smoke or visible fire, I did find water in all three floors within the common areas and in room 310 due to sprinkler activation. WITNESS STATEMENTS: Elayzha L Wilkins- 856-392-8858- Stated that she has a roommate Lezanine Gentile but she was in Newark. She state left an hour before the incident but her room is left unlocked and a group of students usually hang out in her room to smoke. VIDEO SURVEILLANCE: Video cameras were located on the 1st floor of Co-Op Hall. EVIDENCE COLLECTED: Evidence was collected. Bag #DT0005473 was used to store a purple lighter found in the area of origin. Bag # DT0005474 was used to store a plastic bottle with smoking materials found in the area of origin. PHOTOGRAPHS: 47 digital images were captured on during the course of the scene analysis. BUILDING CONSTRUCTION & CONDITION: 2039 Kennedy Blvd Jersey City NJ 07305 is an Educational Facility for Higher Learning the Fire building was located at Co-Op Hall a three story fire- resistive residential building detached on all side. (50 units and 100 occupants). NJ EDUCATIONAL FACILITIES AUTHORITY is listed the owner on the tax records. FIRE PROGRESSION TO OTHER STRUCTURES: The fire did not cause exposure damage. NUMBER OF FATALITIES AND/OR INJURIES: This incident did not result in injuries. INVESTIGATION: The integrity of the scene was maintained by the Jersey City Fire Department during the scene examination. The scene was processed using a systematic approach and working from the unburned to the most burned area. A complete assessment of the exterior and interior of the building was conducted to observe and identify water, smoke and fire damage. On the exterior, signs of water, smoke or fire damage were not found. The interior examination showed the following; water was found within the common area of the first, second and third floor of the Co-Op Hall. 7 door rooms also sustained water damage. The room to 310 was did not show signs of forced entry. Automatic extinguishment system was present. Smoke detectors were present and were operable. AREA OF ORIGIN: The room of origin was located in room 310 of the Co-Op Hall (dormitory). Orientation to the room was as followed: The entrance for room 310 and a twin size bed with the sprinkler head is located on the Alpha side, on the Bravo side wood wardrobes are found, on the Charlie side the area of origin is found under a twin size bed which is again the heating unit to the room which is below the window, on the Delta side wood desks are found. After scene reconstruction: burns, heat and smoke patterns on furniture surfaces and wall indicate that the fire travelled vertically and horizontally from a bottle containing smoking materials and burned the mattress and suitcase which was found in the area of origin . In this area, smoking materials as factors contributing to ignition were located. Electricity to room 310 was still ON and no electrical sources were found in the area of origin. Utilities/ appliances were eliminated as ignition and heat source. Thermal Imaging Camera was utilized to check for hot spots and ensure proper extinguishment. CONCLUSION: The conclusion herein is based on the information available at the time of this report including firefighter and witness interviews, photographs, scene examination and knowledge of fire development. BUILDING CONSTRUCTION AND CONDITION: 222 Mercer St. Jersey City NJ 07302 is a two story Fire resistive building attached on the Bravo side. The building is Ferris High School an educational facility. INVESTIGATION: On Thursday, May 02, 2019 the scene was processed using a systematic approach. My investigation started at the entrance to the Ferris High School Junior academy. I went up to classroom 317 where Ms. Brito pointed xx’s desk. On the desk , an unknown powder residue was present but no burn patterns were found. I then continued to the water fountains outside of classroom 317 and found an unknown powder residue. I then continued to the second floor girl’s bathroom and found unknown black residue. The unknown remains were in common area where other students have access thus evidence was not collected because of contamination and lack of burn patterns. On Friday, May 03, 2019 I returned to Ferris High School and in the Main office I meet with Principal Mr. Jamie Morales, parent xx and xx student. xx provided a statement on her involvement of fire play. She was placed in the Renaissance program by Principal. AREA OF ORIGIN: The area of origin was located room 317. No burn patterns were found within the classroom. Utilities/ appliances were eliminated as ignition and heat source. Thermal Imaging Camera was utilized to check for hot spots and ensure proper extinguishment. CONCLUSION: The conclusion herein is based on the information available at the time of this report including firefighter and witness interviews, photographs, scene examination and knowledge of fire development. Cape May Deputy Chief Troiano received a call on his cell phone from the school's principal, Travis La Ferrier, who stated that a student lit toilet paper on fire in a bathroom and then put it out. The student, an 8 year old boy, was brought into the office where he was extensively questioned before admitting that he set the fire. Principal La Ferrier realized that this student recently had a fire at the house he was living in and called DC Troiano to report his findings. The updated information was relayed to the fire investigator who investigated the fire at the student's house. Arroyo who stated that burn toilet paper was located within the girl’s bathroom on the first floor. Upon our arrival there was no visible smoke or visible fire. WITNESS STATEMENTS: A written statement fromxxxxxx- 15 year old was collected by school Crisis Intervention Teacher Mr. Hampton. Student is a juvenile as a result no statement was collected because the parent was not present. VIDEO SURVEILLANCE: Video cameras were not located. EVIDENCE COLLECTED: Evidence was collected. 1 BiC lighter was in bag # DT0031329 was submitted into evidence. EXHIBITS: 13 digital images were captured on (Panasonic Lumix DMC-TS30) during the course of the scene analysis. The scene was processed using a systematic approach and working from the unburned to the most burned area. A complete assessment of the exterior and interior of the building was conducted. On the exterior, signs of water, smoke or fire damage were not found. No signs of forced entry were present. In the interior, alarms and sprinklers were not activated. The room origin was found on the first floor in the girl’s bathroom. Here, no signs of water, smoke or fire damage were present. In the toilet located in the middle (three toilets were within this bathroom), paper with burn pattern was found. No electrical components were found in the area. Utilities were eliminated as ignition and heat source. A lighter was found by security within the ceiling tiles and submitted into evidence. The lighter was not eliminated as the ignition source. CONCLUSION: The conclusion herein is based on the information available at the time of this report including firefighter and witness interviews, photographs, scene examination and knowledge of fire development. I, Justin Squibb conducted an origin and cause investigation. I spoke to Engine 1132 company officer, Lt. Phil White who directed me to the location of the fire on the Second floor of the school boy’s bathroom. While on the fire floor, I encountered Jim Cummings, Director of Experiential Learning. Mr. Cummings entered the Second-floor hall at approximately 15:01 hours and assisted in putting the fire out using a dry chemical extinguisher. He stated he was notified of a fire in the boy’s bathroom and that he first extinguished the fan cover that was laying on the bathroom floor and then extinguished the fire inside fan housing at the ceiling level. Based on my training, experience and using a scientific method-base investigation, the area of origin is determined to be the bathroom exhaust fan. It should be noted breakers were tripped inside the circuit breaker panel that had a direct link to the exhaust fan. With all other possible ignition sources ruled out this fire is determined to be accidental.
