Ensure that your two programs involved human services agencies, not just health care or mental health. The “conceptual overview’ should be a description of the collaborative efforts, what programs are involved, the mission or purpose stated, unique dynamics or features of the collaborative program, and what benefits the program conceptualizes for their service population/community. Develop a well-informed narrative for each program (2).

What are strengths/benefits and what are the challenges to coordination of care and referring.

Using one of the two programs chosen in part one; assess how this program will provide intervention services to a risk population in need. Please provide a synthesis of the following: Select three programs involved in the collaboration of services model. 1. Identify the title of the program, describe each program - who they serve, where they serve, what type of services and mission. 2. Examine the interventions provided by each program.

Provide a conceptual overview of these two programs. 20 points   2.    Using one of the two programs chosen in #1, assess how this program will provide required resources to at risk population in need. Please provide a synthesis of the following: the at risk population, the resources utilized, and how the chosen referral program assisted in accessing the resources by the at risk population. 30 points   3.    Using one of the two programs chosen in #1, assess how this program will provide required referral to an intervention to serve the at risk population.
