The major issue in Riverbend city is the poor access to medical and preventive healthcare by most citizens. Overall, there is an issue of lack of access to healthcare, which has been a major concern in the city. Thus, the strategic plan intends to ensure increase the number of programs that promote health to Riverbend city citizens to ensure the entire population gains access to preventive care. Preventive care is a priority for the city due to the high rate of chronic illnesses and obesity in the city.

Health disparity has been a major barrier. Discrimination in healthcare has cause some of the population not to receive adequate medical aid and services Thus, the objective of the strategic plan includes; promoting equal access to healthcare, System level intervention in mental health and reducing inequalities in mental health.

Primary Issue to be Addressed

The approaches in the strategic plan seek to address the long-standing issue of health disparities in healthcare within Riverbend city.

They seek to enhance implementation of multiple health promotion programs and health promotion strategies.

It introduces system level interventions that seek to decrease inequalities in mental health.

System level interventions in mental health are crucial in promoting the mental well-being of residents and decreasing inequalities in mental health. Overall, reducing health disparities across the healthcare system will help to reduce health disparities and enhance healthcare outcomes of the population, and patient outcomes for those with mental health conditions.

Methods/Programs/Projects to be Implemented

Use of mobile clinics will enhance reaching of safer and high-quality medical care to patients.

The mobile vans will be equipped with all necessary technology to offer clinical services to underserved areas.

Low-income families will be able to access care services through the Riverbend Care Mobile program

It will enhance the overall health of the city’s population.

Increased access to medical and preventive care will be enhanced through use of mobile clinics to reach the patient at the community level. The mobile clinics will include clinics that offer primary care services from a van equipped with all necessary technology to offer clinical services to underserved areas with Riverbend city. The mobile clinic will be named as “Family Clinic” to enhance cost savings due to allow elderly patients with chronic illnesses and patients with obesity receive care services from the van. The Riverbend Care Mobile Program will provide care services to low-income families across the city through mobile vans that will deliver care at schools and homes removing the barrier of incurring huge transport costs to reach the nearest health facility and incurring huge costs paying for care of family members. It will enhance the overall health of the population since as significant proportion of the population will have access to safer and high-quality health care services (Bertoncello et al., 2020).

Riverbend Mental Wellness Program is targeted at reducing the high prevalence of mental health problems in the city.

It will work with all individuals across the community battling with mental health issues.

It will ensure more residents have access to high-quality mental health services

The Riverbend Mental Wellness program is targeted at helping reduce the rate of mental illnesses within the community and minimize barriers to accessing mental health services. It will aid in reducing the risk of chromic illnesses associated with anxiety, stress, and substance abuse, Most importantly, the mental wellness program will help to save lives, and enhance the mental picture for individuals who may feel hopeless and lost. This program will seek to attend to the needs of residents with mental health problems and work with them through taking positive steps toward recovery (Shalaby & Agyapong, 2020). Maintaining positive mental health within the community will help to stabilize constructive behaviors, thoughts and emotions. In addition, it will aid to address the barriers to mental health care that account for the inequalities in metal health care in Riverbend city.

The source of revenue that will be allocated to support the strategic goal include a grant from the local government.

Other options of funding available include donations and sponsors.

An annual show will be done as a means to get additional funding for the programs.
