https://www.coursehero.com/file/162375314/Proposaldocx. A screenshot of the beginning of page 2 is shown below, and all of this wording (and more, including content and formatting errors) is identical to your submission.

A review of that page's code indicates that this was posted on 8/17, the day you submitted your work to our course. Whether this is work that you took from Course Hero or work that you posted there, this is a clear violation of our Code of Conduct. At this point, the Provost's Office has been notified and will review the work in question to determine if a Code of Conduct violation has indeed occurred. At this time, we wait for their determination. Should it be determined that no violation occurred, the work will be graded without prejudice.

Important: It may help to know that the final grade for the course is a respectable B, whether points are earned by grading or this remains as a zero. The reason this is sent to the Provost's Office is to check the Conduct and to instruct you that using such websites is inappropriate.
