Logic Model Summary

The logic model covers the issue of lack of access to healthcare. The model illustrates how the objectives and outcomes of the program are achieved. Lack of healthcare has been a major concern. The goal of the program is to provide access to preventive healthcare to the population. This issue affects a huge number of people in the society. The resources that will be needed to achieve the desired outcomes and solve the issue in to increase the number of programs that deal with different people in the society especially those with chronic illness and obesity. In addition to providing access to preventive healthcare, the program will also identify and minimize the barriers to healthcare access. This will be done through surveys.

The end goal is to increase access to healthcare to improve the health of the population. Health disparity has been a major barrier. Discrimination in healthcare has cause some of the population not to receive adequate medical aid and services The objective of the program includes; Equal access to Healthcare, System level intervention in mental health and reducing inequalities in mental health. The approaches that will be taken to achieve the outcomes are; Create more programs for chronic disease and obesity prevention, reducing health disparities aids and improving healthcare outcome of the population and improving the health outcomes of patients with mental health conditions
