1. Download the current VR1850 from the TWC forms website. 2. Complete all 11 sections on yourself. We understand the confidentiality of information, but please

ensure you complete the form with the required information needed for an application and/or resume. All information must contribute to filling out an application, creating a resume for the purpose of obtaining employment

3. Do not check “no” for paid work history or volunteer history. If you do not have any volunteer history, research an agency in your area in need of volunteers and use their information.

4. Provide 12 Employment Skills. The expectation is that you will be required to provide 12 for your customers, you have to be able to provide 12 for yourself here.

5. Do not check “no” for Training History, or Occupational License or Certificate, if you do not possess license please research a license and provide information. (NOTE: UNTWISE credentials are not "training's") All individuals in this course possess a certification.

6. List 3 professional references. 7. Do not use fictitious information for this assignment, for example Disney/cartoon characters, "John

Doe", etc...no points will be awarded for these types of entries.
