Explain the medication reconciliation process

The process of medication errors results in many adverse drug events and death. “The process of

medication reconciliation obtaining, verifying, and documenting a list of the patient’s current

medicines and then comparing the list with the medication order and the patient condition to help

identify and resolve any discrepancies” (Vallerand, & Sanoski, 2020). Erroneous drug histories

may lead to undesired patient outcomes or leading to adverse consequences. The patient must

continue all the home medication except metformin; however, there is no substitute for this

medication which puts the patient in great danger of increased blood sugar.

Analyze the client scenario for any omissions, additions, or duplications of medications and

discuss the implications to the client.

In this case, the patient is required to continue all the home medication except metformin.

Therefore, metformin is omitted in this case. This vital medication may be used alone or together

with other medicines in treating type 2 diabetes. This drug helps control glucose in the blood by

decreasing the amount of glucose that a person absorbs from the food taken. This medication,

therefore, reduces the symptoms and the risks of high blood sugar. This means that stopping this

medication may have adverse effects on this patient. The short and long-term impact on the

blood sugar going up until the drug is not substituted with anything else.

Analyze the client scenario for potential risks associated with the client’s current condition

and currently prescribed home medications

I. Insulin subcutaneous based on sliding scale B in patients with type 2 diabetes may lead to

enhanced glycaemic control in the short term. Still, they may, in the long term, increase

the risk for severe hypoglycaemic episodes. This increases the risk of dementia for the

patient.

II. Omission of metformin may lead to increased blood sugar which puts the patient at an

increased risk of blurred vision, fatigue, and thirst.

III. The patient is also at an increased risk of anxiety and depression due to high blood sugar.

Identify potential drug interactions and prioritize the drug interactions that are most

concerning to you.

I. Drug interactions are reported among people taking metoprolol and piperacillin. Some of

the effects include chest pains, dehydration, chronic kidney disease, anemia, and cerebral

infarction (Bailie, 2021).

II. Another possible drug interaction is Enoxaparin and Metropol. The common drug

interactions include fatigue in females and vomiting among male patients.

III. There is another possible drug interaction of Enoxaparin and garlic. These two increase

the risk of side effects to the patient and may also cause bleeding.

IV. Enoxaparin and Piperacillin increases the risk of blood albumin among the females and

diarrhea among the males

V. The drug interaction that is very concerning to me is metoprolol and piperacillin. This is

because the two drugs have the potential for severe interaction and require regular

monitoring. The healthcare provider may be necessary to use alternative medication to

avoid this kind of interaction.

Identify actions that the nurse should implement and provide a reason for each action.

a. To prevent drug interaction, the nurse should review the medication guide,

prescription information, and drugs fact labels with every new prescription.

Review the information frequently is very important because the labeling may

change with new medication information. By learning about the possibility of

drug interaction, the nurse should talk with other healthcare providers about the

possibility of an alternate medication. With proactive efforts, drug interactions are

usually preventable.

b. It is also essential for the nurse to ensure a free and effective flow of information

from one healthcare provider to the other. Communication is vital between the

nurses, pharmacists, and other healthcare workers involved in this case.

Therefore, the nurse should ensure effective communication that will help ensure

minimal or no medical errors.

c. It is also essential that the patient is educated about the various effects of the

drugs and the drug interactions. Safe medication administration will help to

enhance patient outcomes and reduce adverse effects.
