Presenting Problem or Concern:

Almeda is a teenage mother that has been dealing with the pressures of being a single young mother. Almeda is struggling with being a young parent and helping your own parents as well. Almeda at a young age had to deal with adulthood while still being a child herself. Alameda does not have the life skills she needs to take care of the responsibilities she has as far as her child. Alameda lacks social skills, struggles with basic parenting skills.

Almeda comes from a two-parent home. Both of Almeda parents is undocumented. Almeda is the sole supporter of her family. Almeda parents does not support her due to the lack of support they have for themselves. Almeda parents are afraid of seek help from social agencies due to being undocumented workers.

Environmental Supports:

Almeda lacks support in housing, food, childcare services, and education. Almeda is working with a case worker that is connecting the client with services such as teenage support groups for teenage moms. Almeda case worker is the process of finding adequate housing for Almeda and her child.

Client Functioning in the Environment:

Almeda social and emotional skills social awareness. Though she is social, the client lacks basic cognitive skills.

Personal History of Trauma/Abuse/Neglect:

Almeda has a history of dealing with poverty and social oppression. Almeda has been dealing with poverty from an early age, no adequate housing, and lack basic social skills.

What Gives Client’s Life Meaning:

Almeda meaning of life is her child that she care for.

Need for Services:

Almeda needs services in the following areas: education, housing, childcare services, and counseling services. The presenting problem is Almeda age only being 17 years old. Almeda to be referral to teenage support group, contact the clients to a teenage housing facility for her and her child. The clients need referrals to SNAP benefits and well as childcare services.
