· Graduate academic records

· Cover letter

· Current résumé

· Three Letters of Recommendation (one from a faculty member if currently enrolled in a graduate degree program)

· Narrative statement of fellowship and career objectives. Include in your statement answers to the following:

. What would you bring to Trinity Health to help us better live out our mission?

. How would the fellowship help you achieve your career goals?

· Email and telephone numbers where you can be reached in 2022

· Desired Fellowship location

· Fellows selected for interviews will participate in the virtual interviewing the week of October 24, 2022. You will have a chance to learn more about Trinity Health and speak with the preceptor for the fellowship area where you applied.
