Theories of Accident Causation
Theory Summary and Why
The domino theory of accident causation was one of the earliest developed. According to this theory, there are five factors in the sequence of events leading to an accident: ancestry/social environment, fault of person, unsafe act/mechanical or physical hazard, accident, and injury. According to Heinrich, fault of person is inherited because of their social environment by ancestry. He also emphasized that incidents, not injures or property damage, should be the point of attack. For instance, if person slips or may fall, an injury may or may not results but there is an accident taken place.

Summary of Accident
An employee of Behr Iron & steel Inc. was involved in an accident while on work and lost his life. The company failed to provide protection as it is required under OSHA regulations for company’s employees who were cleaning a shredder discharge pit. There was no emergency shut off switch. The company’s admitted that the cause of their employee’s death was due to their own violations of failing to provide protection to their employees. The company faces a maximum sentence of five years’ probation and maximum fine of $500,00, and also restitution to the victim employee in an amount determined by court.
Relation Between Theory and Accident
This theory provides that there are five factors that can lead to an accident. One of this faults is the fault of persons. Mechanical hazard an, accident and injury. According to this article, there are three things that led to the death of this employee. The first one is fault of person, there was somebody in the company that failed to provide protection to the workers, secondly, the mechanical injury in which the employee was involved in and finally, the injury was accidental even though the accident could have been avoided if the company was responsible enough.
