Behavior Plan Template

Use this template to create behavioral plans for assignments in Weeks Two and Three. Fill in each section and provide additional information as needed.

History

Describe all pertinent and known history.

Medical

The client has no known medical history but suffers from low self-esteem.

Family

The client’s died at the age of 10. The father is an alcoholic the other relatives have less concern about him.

Substance Abuse

The client has a history of alcohol use and drug substance use. Alcohol use potentially learnt from the father who suffers from alcoholism.

Developmental and Social

The client lived with his mother until age 10 when she died. Since then, the client has lived in transient shifts with the father and relatives. The client has a low self-esteem due to poor treatment by relatives after mother’s death.

The client also has a chronic stealing behavior which he is proud about. The client’s has not completed secondary education or GED and reports an unsteady employment.

Course of Treatment

List two target behaviors and two interventions.

Target Behaviors

The client will abstain from substance abuse for the next 6 months

The client will complete his GED in the next one and a half years.

Frequency and Duration of Target Behaviors

Daily basis

Attending GED classes at least four times a week.

Severity and Number of Relapses

An accommodation of twice per month.

Once per fortnight.

Interventions

The client will be educated comprehensively on the negative implications of substance abuse and stealing.

The client will be relocated to an environment with a low prevalence of substance abuse

The client will be enrolled in GED classes and registered to sit for the examination.

The client will educated on the importance of responsible living or oneself and encouraged and helped to source for a part-time employment to meet his needs and pay for the GED classes.

Goals and Objectives

List two long-term goals, two short-term goals, and one objective to work towards each goal.

Long-Term Goal

Completion of high school education.

Enrollment in college or obtaining a sustainable source of income.

Short-Term Goal

Short term Goal 1

1. Developing a sense of purpose in life, increase his elf-esteem, and develop effective social skills.

2. Overcoming substance abuse and stealing behaviors.

1. To enable the client have a different outlook of life and exposure

1. To enable the client lead a responsible life in the future.

1. To help the client live objectively and purposefully.

1. Enable the client to appreciate the dangers of substance abuse and stealing behavior.

Discharge and Termination Plans

Describe the discharge and termination plan for the client.

Discharge Plan

Client will be discharge upon the successful achievement of the short term goals in this plan. A follow up plan will be enacted where the client will be required to report to the facility once per 2 weeks for a period of 6 months. If the client maintains the prescribed behavior, he will be allowed to report to the facility once per month thereafter till termination. However, this may be adjusted based on the client’s performance and institution schedule.

Termination Plan

Termination will be enacted after the client has successfully achieved both long term goals and seen to be capable of leading a responsible life thereafter. Genuine transformation of behavior will have to be showcased before termination.
